研究目的：

探讨围手术期应用氨甲环酸联合咀嚼口香糖方案既能有效减少手术患者围手术期失血，还可以改善患者胃肠功能紊乱，同时减少术后输血率、感染率、并发症发生率，缩短术后下床时间，促进患者早期功能锻炼，加速术后康复，减少平均住院日和医疗费用。本研究拟采用围手术期应用氨甲环酸联合咀嚼口香糖的方法，探讨其在骨科手术中减少围手术期出血并同时改善胃肠功能紊乱的安全性及有效性。  

Objectives of Study：

To explore the application of tranexamic acid combined with chewing gum in perioperative period can not only effectively reduce the perioperative blood loss of patients, but also improve the gastrointestinal dysfunction of patients. At the same time, it can reduce the rate of postoperative blood transfusion, infection rate and complication rate, shorten the time of getting out of bed after operation, promote early functional exercise of patients, accelerate postoperative rehabilitation, and reduce the average hospitalization days and medical expenses. This study intends to explore the safety and effectiveness of perioperative application of tranexamic acid combined with chewing gum in orthopedic surgery to reduce perioperative bleeding and improve gastrointestinal dysfunction.

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1）凡不为上述诊断的患者皆于研究范围之外； 2）术前贫血（Hb：女<110g/L，男<120g/L）； 3）有动静脉血栓栓塞性疾病史，包括脑血管意外、深静脉血栓（DVT）形成、肺栓塞或既往行支架介入术或其他手术，术后长期服用抗凝药物患者； 4）有使用TXA禁忌，包括支架介入术后患者、血栓形成倾向的房颤患者； 5）对TXA过敏者； 6）凝血功能障碍，术前血小板<100*10^9L，国际标准化比值（INR）>1.4，或活化部分凝血酶时间（APTT）>1.4倍正常值； 7）严重心、肺、肝、肾功能不全者。

Exclusion criteria：

1) All patients with no such diagnosis were excluded from the study; 2) Preoperative anemia (Hb: female <110g/L, male <120g/L); 3) Patients with a history of arteriovenous thromboembolism, including cerebrovascular accident, deep vein thrombosis (DVT) formation, pulmonary embolism, or previous stent intervention or other operations, and long-term use of anticoagulant drugs after surgery; 4) TXA is contraindicated, including patients after stent intervention and patients with atrial fibrillation who are prone to thrombosis; 5) People allergic to TXA; 6) Coagulation dysfunction, preoperative platelet <100*10^9L, International standardized ratio (INR) >1.4, or activated partial thrombin time (APTT) >1.4 times the normal value; 7) Serious heart, lung, liver, renal insufficiency.
