研究目的：

PJS患者早期行小肠镜下多发息肉切除是安全有效的，且可有效避免外科手术的风险。无痛小肠镜可以抑制操作时对患者的刺激，可以防止出现体动、呛咳等影响检查操作的不良反应，并且可以提高患者的舒适度和满意度。本研究拟通过观察环泊酚用于PJS患者行无痛小肠镜治疗的诱导及维持的效果，探索可以降低或减少经口小肠镜在全麻诱导及维持过程中发生循环波动的管理办法，从而避免PJS患者诊治过程中由于循环波动大引发的心、脑血管意外、心律失常的发生，避免心脏骤停、循环衰竭等严重并发症的发生。同时观察和分析环泊酚对术后胃肠功能影响及诱导注射痛的发生情况以及对术后认知功能的影响，寻找能更好地降低患者术后胃肠道不良事件的发生、减轻诱导注射痛及避免术后认知障碍的药物，提高PJS患者行小肠镜诊治围术期的安全性及舒适度。  

Objectives of Study：

Early resection of multiple polyps under enteroscopy is safe and effective for patients with Peutz-Jeghers syndrome (PJS), and can effectively avoid the risks of surgical operations. Painless enteroscopy can suppress the stimulation to patients during the operation, prevent adverse reactions such as body movements and choking coughs that may affect the examination operation, and improve the comfort and satisfaction of patients. This study aims to observe the induction and maintenance effects of Ciprofol in the treatment of PJS patients undergoing painless enteroscopy, and explore management methods that can reduce or minimize the occurrence of circulatory fluctuations during general anesthesia induction and maintenance in transoral enteroscopy. This is to avoid the occurrence of cardiovascular and cerebrovascular accidents, arrhythmias caused by large circulatory fluctuations during the diagnosis and treatment of PJS patients, and prevent serious complications such as cardiac arrest and circulatory failure. At the same time, observe and analyze the effects of Ciprofol on postoperative gastrointestinal function, the occurrence of injection pain during induction, and the impact on postoperative cognitive function. Search for drugs that can better reduce the occurrence of postoperative gastrointestinal adverse events, alleviate injection pain during induction, and avoid postoperative cognitive impairment, so as to improve the safety and comfort of PJS patients during the perioperative period of enteroscopy diagnosis and treatment.

纳入标准：

Inclusion criteria

排除标准：

1.存在长期服用阿片类药物情况； 2.合并严重心脏、肝脏、肾脏等疾病； 3合并糖尿病并发高血糖所致脏器损害； 4.近期有重大手术经历； 5.有面部严重畸形、颞颌关节强直、强制性脊柱炎等气道管理困难； 6.咽喉部存在感染或肺顺应性降低，呼吸道出血； 7.有呕吐、反流、误吸或高度危险，如未禁食、饱胃、腹内压过高、怀孕超过14 周、肠梗阻、食管裂孔疝； 8.不具备自理能力，非完全行为能力人； 9.存在抗拒小肠镜检查行为； 10.严重过敏史、精神疾病、心理疾病患者。

Exclusion criteria：

1. Have a history of long-term use of opioid drugs; 2. Be complicated with severe diseases of the heart, liver, kidney, etc.; 3. Be complicated with organ damage caused by hyperglycemia in patients with diabetes mellitus; 4. Have experienced major surgeries recently; 5. Have difficulties in airway management, such as severe facial deformities, temporomandibular joint ankylosis, ankylosing spondylitis, etc.; 6. Have infections in the throat, decreased lung compliance, or respiratory tract bleeding; 7. Have a high risk of vomiting, regurgitation, and aspiration, such as not fasting, a full stomach, excessive intra-abdominal pressure, pregnancy over 14 weeks, intestinal obstruction, and hiatal hernia; 8. Lack the ability to take care of themselves and are not fully capable of civil conduct; 9. Show resistance to enteroscopy examination; 10. Be patients with a history of severe allergies, mental diseases, or psychological disorders.

Blinding：

Each patient's corresponding random number was placed into an opaque envelope, and based on the random numbers, they were assigned to receive either propofol or ciprofol. Both the patients and the anesthesiologists responsible for data collection were unaware of the group assignments. The gastroenterologists, other healthcare providers, outcome assessors, and the patients themselves were all blinded to the intervention.
