研究目的：

1.探明广州市社区中房颤患者规范抗凝的变化趋势及其影响因素。 2.评估多学科协作的综合管理模式对优化患者治疗依从性、改善生活质量和降低疾病负担中的作用。 3.选择试点社区，探索数字化健康行为干预的方式（如短信提醒）在提高房颤患者抗凝治疗依从性及自我管理中的可行性和有效性。  

Objectives of Study：

1. To identify the trend of standardised anticoagulation for patients with atrial fibrillation and the factors influencing it in the Guangzhou community. 2.To assess the role of multidisciplinary integrated management model in optimising patients' adherence to treatment, improving quality of life and reducing the burden of disease. 3. To select pilot communities to explore the feasibility and effectiveness of digital health behaviour interventions (e.g., SMS reminders) in improving anticoagulation adherence and self-management of patients with atrial fibrillation.

纳入标准：

Inclusion criteria

排除标准：

1.年龄 < 65岁，不符合老年人体检项目标准的个体。 2.没有明确的房颤诊断，仅表现为孤立性心房扑动的患者。 3.患有无法配合研究随访的严重疾病（如终末期癌症、重度心力衰竭、严重精神疾病）或预期生存期低于1年的个体。 4.居住在外地或有频繁迁移计划，预计无法完成整个随访周期的患者。 5.目前正在参加干预性临床试验的患者，以避免研究间的相互干扰。

Exclusion criteria：

1. Individuals < 65 years of age who do not meet the criteria for the Geriatric Physical Examination Programme. 2. Individuals who do not have a clear diagnosis of atrial fibrillation and only have isolated atrial flutter. 3.Individuals with a serious medical condition (e.g., end-stage cancer, severe heart failure, severe mental illness) that precludes cooperation with study follow-up or with an expected survival of less than 1 year. 4.Patients who reside out of state or have frequent relocation plans and are not expected to complete the full follow-up cycle. 5. Patients currently enrolled in an interventional clinical trial to avoid inter-study interference.
