研究目的: |
1)分析4000例18 - 60岁健康汉族职业性噪声暴露1年以上的人群(船厂工人),根据其噪声暴露、其他环境因素暴露、生活行为方式和听力学检查等数据,结合测量到的现场噪声暴露水平及特征,分析噪声性听力损失的临床表型和影响因素因素,获得NIHL的临床流行病学数据和听力学特征,尝试建立噪声暴露、年龄等主要影响因素与听力表型的预测模型,为NIHL的发病过程及早期预防提供指导。(部分已完成) 2)对样本进行听力表型的细化分层,通过调查问卷信息、外耳、中耳检查和听力学特征排除年龄、遗传、外伤、疾病等其他致听力损失因素的干扰后,筛选出同等噪声暴露情况下,噪声导致高频听力损失程度最严重的样本和听力损失程度最低的样本,经过严格匹配对照后分别作为NIHL易感组和抵抗组,通过对其血液样本抽提DNA进行全外显子二代测序分析,比较组间基因表达的差异,筛选出与NIHL高度相关的候选基因热点突变型,结合生物信息学统计分析,发现NIHL的易感基因和/或抵抗基因。 3)根据前期的听力表型分类,针对上述前期工作中二代测序分析筛选出的候选基因突变型(20个以内)加之申请者团队此前感兴趣的GJB2基因p.V37I突变型,对工人的血液DNA进行SNP测序,了解这些相关突变型在汉族人群中的携带率,并实现在大样本中验证相关突变型与噪声性听力损失的相关性,为相应基因致病机制的基础研究以及未来基因筛查诊断提供人群依据。 |

Objectives of Study: |
1. Obtain the noise exposure, life behavior patterns and auditory data of 4000 healthy Chinese Han people (shipyard workers)aged 18 to 60 with occupational noise exposure for more than 1 year, combined with measured on-site noise exposure level and characteristics, to analyze the clinical phenotype, influencing factors, epidemiology data and the audiology characteristics of NIHL, try to establish the predictive model of NIHL related to the noise exposure, age and other major influencing factors, and provides guidance on the early stage prevention of NIHL. (partially completed); 2. Subdivided the subjects into different audiological phenotype groups, according to the questionnaire information and audiological characteristics, excluding the influence of gender, age, trauma, disease and other factors may contribute to hearing loss. We would screen out the most serious hearing loss subjects and those strictly matched subjects without any noise induced hearing loss from the same noise exposure level population as NIHL susceptibility and resistance groups respectively. To compare the differences of genetics susceptibility between two groups, we would use the DNA samples extracted from those peoples peripheral blood to obtain the whole exome sequence by next-generation sequencing technique, and analyze the candidate gene mutations highly correlated with NIHL through bioinformatics statistical analysis, and finally found the susceptibility gene or resistance gene of NIHL; 3. According to the previous audiological phenotype categories, DNA samples extracted from the blood of these workers would be sequenced to get those candidate gene mutations screened from the whole exome sequencing analysis in previous work and our interest mutant GJB2 p.v37i data, to understand the mutation rates of these correlated mutations in the Han population, and to verify the correlation between the correlated mutations and the noise hearing loss in large samples, which provides a basis for the animal study of the corresponding gene pathogenesis and the future gene screening diagnosis. |
