研究目的：

通过观察不同剂量阿芬太尼联合丙泊酚在不使用肌松药喉罩全麻下小儿支气管镜检查中的应用，并对比芬太尼联合丙泊酚使用的效果及相关不良事件的发生率，探寻麻醉效果好且不良反应少的阿芬太尼适宜用药剂量，以期为临床提供一种新型的、安全性高、舒适性好、苏醒快的无痛支气管镜诊疗方案。  

Objectives of Study：

By observing the application of different doses of alfentanil combined with propofol in pediatric bronchoscopy under laryngeal mask anesthesia without muscle relaxants, and comparing the effects of fentanyl combined with propofol as well as the incidence of related adverse events, this study aims to explore the appropriate dosage of alfentanil that provides good anesthetic effects with few adverse reactions, in order to provide a novel, highly safe, comfortable, and rapid-recovery painless bronchoscopy treatment regimen for clinical practice.

纳入标准：

Inclusion criteria

排除标准：

a.有先天性遗传病； b.癫痫或精神疾病； c.严重心肺和肝肾疾病； d.可预见的困难气道； e.对研究药物的过敏史； f.检查前72小时服用任何类型的催眠或镇痛类药物。

Exclusion criteria：

a. Congenital genetic disorders b. Epilepsy or psychiatric conditions c. Severe cardiopulmonary, hepatic, and renal diseases d. Anticipated difficult airway e. History of allergy to study medications f. Administration of any type of hypnotic or analgesic medications within 72 hours prior to the examination.

A1组先缓慢静脉注射(约 30s)阿芬太尼 5ug/kg，2min 后给予丙泊酚 2mgkg，待患儿睫毛反射消失后置入喉罩，连接T型管和麻醉环路，采用压力控制同步间歇指令通气。氧流量为 3Lmin，吸入氧浓度为 70%，吸气压15cmH0，通气频率 10~12 次/min，维持 PetCO,35~45mmHg。支气管镜进入声门和到达气管隆突时喷酒 2%利多卡因进行表面麻醉。

The A1 group was first slowly injected with 5ug/kg of alfentanil intravenously (about 30s), followed by 2mg of propofol 2mgkg after 2 minutes, and then placed in a laryngeal mask after the eyelash reflex disappeared, connected to the T-tube and anesthesia loop, and used pressure control to synchronize intermittent command ventilation. The oxygen flow rate is 3Lmin, the inspired oxygen concentration is 70%, the inspiratory pressure is 15cmH0, the ventilation frequency is 10~12 times/min, and the PetCO is maintained, 35~45mmHg. Bronchoscopy enters the glottis and reaches the tracheal carina with 2% lidocaine for topical anesthesia.
