研究目的: |
目的一:建立急慢性意识障碍患者的多阶段多模态数据集和特征集。具体如下:1.验证“时-频-空”意识神经机制和意识水平度量方法:利用神经影像分析意识缺失的关键脑区和重要脑环路,时-频-空脑电特征解释潜在的意识障碍患者脑功能异常,验证“时-频-空”意识神经机制和意识水平度量方法。2.建立面向脑功能的急慢性意识障碍患者意识检测体系:根据意识相关的特征,选取检测方法,整合多种方法的优势构建体系,进行临床验证。3.建立基于脑功能特征的急慢性意识障碍临床预后模型:利用人工智能算法从意识相关的特征选择与临床预后和苏醒相关的特征,单特征和多特征组合构建机器学习模型。4.建立急慢性意识障碍患者临床预后和苏醒相关的脑功能检测标准:验证预测模型准确性,信效度检验,制定相关标准。 目的二:验证多物理场联合调控在意识障碍患者中的治疗效果。具体如下:1.进行系统化临床实验,包含不同意识障碍分型患者,为患者制定多物理场联合调控治疗方案,主要包括磁刺激、电刺激和超声刺激等多种调控手段的联合应用。2.针对不同干预手段的效果,对患者行为反应量表进行定期测评,全面追踪患者神经功能和意识水平的动态变化。收集多模态生理数据,如脑电活动、血氧水平、心率变异性等,捕捉调控后患者神经网络活动的潜在变化与功能恢复的特征。3.制定急慢性意识障碍新型干预手段临床应用专家共识,建立多物理场联合调控系统操作临床应用方案,制定操作规范,撰写相关专家共识。 |

Objectives of Study: |
Objective 1: To establish a multi-stage, multimodal dataset and feature set for patients with acute and chronic disorders of consciousness. The specifics are as follows: 1. Verification of "Time-Frequency-Space" Neural Mechanisms of Consciousness and Methods for Measuring Consciousness Level: Using neuroimaging analysis to identify key brain regions and important brain circuits related to loss of consciousness, and interpreting the abnormal brain function in patients with consciousness disorders through time-frequency-space EEG features. This will verify the "time-frequency-space" neural mechanisms of consciousness and methods for measuring consciousness level. 2. Establish a Brain Function-Based Consciousness Detection System for Acute and Chronic Disorders of Consciousness: Based on consciousness-related features, select detection methods and integrate the advantages of various methods to construct the system, followed by clinical validation. 3. Establish a Clinical Prognostic Model for Acute and Chronic Disorders of Consciousness Based on Brain Function Features: Use artificial intelligence algorithms to select features related to consciousness, clinical prognosis, and awakening, constructing machine learning models with both single and combined features. 4. Establish Clinical Prognostic and Awakening-Related Brain Function Detection Standards for Acute and Chronic Disorders of Consciousness Patients: Verify the accuracy of predictive models, conduct reliability and validity testing, and establish relevant standards. Objective 2: To validate the therapeutic effects of multiphysical field combined regulation in patients with consciousness disorders. The specifics are as follows: 1. Conduct Systematic Clinical Experiments, Including Patients with Different Types of Consciousness Disorders: Develop treatment plans combining multiple physical fields, including magnetic stimulation, electrical stimulation, and ultrasound stimulation, among other regulatory methods. 2. Assess the Effects of Different Intervention Methods: Regularly evaluate patient behavior response scales, comprehensively track dynamic changes in patients' neurological function and consciousness levels. Collect multimodal physiological data, such as EEG activity, blood oxygen levels, and heart rate variability, to capture potential changes in neural network activity and features related to functional recovery after interventions. 3. Develop Expert Consensus on New Intervention Methods for Acute and Chronic Disorders of Consciousness: Establish clinical application guidelines for the multiphysical field combined regulation system, create operating standards, and compile the relevant expert consensus. |

急性意识障碍患者排除标准: 1.既往有精神系统疾病史,有听力障碍病史;2.颅内有动脉夹,起搏器等金属植入物;3.试验期间服用镇静剂等影响皮层兴奋性的药物;4.存在不可控制的癫痫或不自主运动;5.无自主呼吸;6.癫痫持续状态;7.需要紧急神经外科手术治疗等无法配合检查。 慢性意识障碍患者排除标准: 1.既往有精神系统疾病史,有听力障碍病史;2.颅内有动脉夹,起搏器等金属植入物;3.实验期间服用镇静剂等影响皮层兴奋性的药物;4.存在不可控制的癫痫或不自主运动。 |

Exclusion criteria: |
Exclusion Criteria for Patients with Acute Impairment of Consciousness: 1. History of psychiatric diseases and hearing impairment in the past; 2. There are metal implants such as arterial clips and pacemakers in the skull; 3. Taking sedatives and other drugs that affect the excitability of the cortex during the test; 4. Presence of uncontrollable epilepsy or involuntary movements; 5. No spontaneous breathing; 6. Status epilepticus; 7. Unable to cooperate with the examination due to urgent neurosurgical treatment. Exclusion Criteria for Patients with Chronic Impairment of Consciousness: 1. History of psychiatric diseases and hearing impairment in the past; 2. There are metal implants such as arterial clips and pacemakers in the skull; 3. Taking sedatives and other drugs that affect the excitability of the cortex during the experiment; 4. Presence of uncontrollable epilepsy or involuntary movements. |
