研究目的：

1.构建基于接纳与承诺疗法的慢性心衰患者抑郁管理方案 2.验证HeartACT效果：应用开发的再入院风险在线网页计算器，识别高风险人群开展ACT干预，通过结局指标慢性心衰患者的抑郁评分（主要结局指标）、再入院率、焦虑评分、心理灵活度、认知融合、睡眠质量（次要结局指标），明确基于接纳与承诺疗法的健康干预在改善慢性心衰患者抑郁症状和30天再入院率的效果。 3.评价经济学价值：干预结束后，结合试验结果，从经济学的角度分析HeartACT干预模式是否比同期常规干预更具有优势。明确其的经济学价值和现实意义，为进一步的推广和应用提供策略支持。  

Objectives of Study：

1.Develop a depression management program for patients with chronic heart failure based on Acceptance and Commitment Therapy. 2.Verify the effectiveness of HeartACT: Use the developed online web calculator for readmission risk to identify high-risk groups for ACT intervention. Through outcome indicators such as depression scores (primary outcome indicator), readmission rate, anxiety scores, psychological flexibility, cognitive fusion, and sleep quality (secondary outcome indicators) of patients with chronic heart failure, clarify the effect of health intervention based on Acceptance and Commitment Therapy in improving depressive symptoms and 30-day readmission rate in patients with chronic heart failure. 3.Evaluate economic value: After the intervention, combined with the trial results, analyze from an economic perspective whether the HeartACT intervention model has more advantages than the conventional intervention in the same period. Clarify its economic value and practical significance, and provide strategic support for further promotion and application.

纳入标准：

Inclusion criteria

排除标准：

1.其他心脏疾病者； 2.合并新发急性冠脉综合征、恶性心律失常等导致CHF急性加重者； 3.肝肾功能疾病者； 4.进行性神经功能缺损(如进展性肌无力、感觉异常等)； 5.已确诊精神心理障碍，正在接受心理治疗者； 6.有药物或酒精依赖史； 7.正在参与其他研究者。

Exclusion criteria：

1. Patients with other heart diseases; 2. Patients with acute exacerbation of CHF caused by new-onset acute coronary syndrome, malignant arrhythmia, etc.; 3. Patients with liver or kidney function diseases; 4. Patients with progressive neurological deficits (such as progressive muscle weakness, paresthesia, etc.); 5. Patients with confirmed mental and psychological disorders who are receiving psychological treatment; 6. Patients with a history of drug or alcohol dependence; 7. Patients currently participating in other studies.

对照组接受所在干预中心心内科提供的常规护理。出院时，患者接受常规出院指导，内容包括心理、氧疗、饮食、用药指导和服药管理指导等，提醒患者出院后及时复诊。
