研究目的：

肛痈（肛周脓肿）是一种肛周常见的感染性疾病，可沿肛周间隙发展、蔓延。明确脓肿位置、彻底引流是治疗关键。引流术后，约有7%~66%的患者发生肛瘘；为降低肛瘘形成率，部分学者在引流时对可疑瘘管切开或挂线，称之为“根治性脓肿手术”。该手术术后肛门缺损或失禁的可能性大；且患者在引流术后有一部分不发生肛瘘，所以对全部患者行括约肌的切开缺乏理论依据。为使该病得到有效治疗，并且更好保护肛门功能，我们根据其发病的病理生理机制，在引流时将粘膜下间隙、括约肌间隙和外括约肌以外间隙充分切开引流，即“肛周脓肿三间隙引流”（three cavity clearance,TCC），术中充分引流潜在感染间隙，并行全括约肌保留，减少对肛门功能的损伤及术后疾病的复发、肛瘘的发生等。  

Objectives of Study：

Anal fistula (perianal abscess) is a common perianal infectious disease that can develop and spread along the perianal space. Clear abscess location, thorough drainage is the key to treatment. After drainage, about 7% to 66% of patients with anal fistula; to reduce the incidence of anal fistula, some scholars in the draining of suspected fistula incision or hanging line, called "radical abscess surgery." The postoperative anal defect or incontinence is highly likely; and the patient does not have anal fistula after drainage, so there is no theoretical basis for sphincterotomy in all patients. In order to get effective treatment of the disease and better protect the anal function, according to its pathophysiological mechanism, we will fully drain and drain the submucosal space, the sphincter space and the space outside the external sphincter during drainage, that is, "three perianal abscesses "(Three cavity clearance, TCC), the full draining of potential infection during surgery, the parallel reservation of the whole sphincter to reduce the damage to anal function and postoperative disease recurrence, the occurrence of anal fistula and so on.

纳入标准：

Inclusion criteria

排除标准：

（1）妊娠和哺乳期妇女；
（2）结核、克罗恩病、结直肠肿瘤等特异性肛周脓肿；
（3）伴有糖尿病等可能影响创面愈合者；
（4）合并免疫缺陷疾病、脊髓及盆底疾病、精神方面疾病者；
（5）既往有脓肿手术史，并出现肛门失禁、感觉异常者；
（6）依从性差。

Exclusion criteria：

(1) women in pregnancy and lactation;
(2) specific perianal abscess, such as tuberculosis, Crohn's disease, colorectal tumor, etc.;
(3) patients with diabetes, such as diabetes, may affect the wound healing;
(4) the combination of immunodeficiency diseases, spinal cord and pelvic floor diseases and mental diseases;
(5) a history of abscess surgery, and anus incontinence and abnormal sensation of the anus;
(6) poor compliance.
