研究目的：

确定适合我国国情男性前列腺癌筛查方案，为制订可全国推广的筛查方案提供科学依据。 1）临床研究 评估开展前列腺特异抗原（PSA）筛查对我国男性前列腺癌发病和死亡的影响，评价PSA筛查的近期和长期效果； 2）卫生经济学评价 评价基于PSA的筛技术应用于大规模筛查项目的卫生经济学效果，分析其成本效果、成本效益，建立卫生经济学决策分析模型。  

Objectives of Study：

To determine a prostate cancer screening program that is suitable for the national conditions of China, and to provide a scientific basis for the development of a screening program that can be widely implemented across the country. 1) Clinical Research: To evaluate the effect of prostate-specific antigen (PSA) screening on the incidence and mortality of prostate cancer in Chinese men, and to assess the short-term and long-term effects of PSA screening. 2) Economic Evaluation: To evaluate the economic effects of applying PSA-based screening technologies in large-scale screening projects, to analyze the cost-effectiveness of this screening program, and to establish health economic decision analysis models.

纳入标准：

Inclusion criteria

排除标准：

1.有癌症疾病史，或接受过任何种类的癌症相关治疗（非黑色素瘤皮肤癌除外）； 2.2年内进行过PSA检查； 3.患有其他严重疾病，可能会影响筛查获益（如脑卒中、严重肺病、终末期肾病、终末期肝病、严重心力衰竭等）； 4.48h内有过直肠指诊、膀胱镜检查、导尿等操作； 5.24h内有过射精行为； 6.患前列腺炎，或曾患前列腺炎但治愈未超过两周； 7.服用前列腺增生药物（如非那雄胺片等）。

Exclusion criteria：

1. History of cancer or received any type of cancer-related treatment (except for non-melanoma skin cancer); 2. PSA test conducted within the past 2 years; 3. Suffering from other serious illnesses that may affect the benefits of screening (such as stroke, severe lung disease, end-stage kidney disease, end-stage liver disease, severe heart failure, etc.); 4. Rectal examination, cystoscopy, catheterization or similar procedures performed within the last 48 hours; 5. Ejaculation occurred within the last 24 hours; 6. Suffering from prostatitis, or had prostatitis but cured for less than two weeks; 7. Taking medications for benign prostatic hyperplasia (such as finasteride, etc.).

采集静脉血，用于干预组PSA筛查检测。用于化验检测的生物学标本应按要求妥善保存，在规定的时间内完成检测，并及时对PSA阳性的个体进行通知，建议进一步推荐前往相关科室进行复检和诊治。 筛查组基线进行1次PSA检测，PSA>=4.0ng/ml者，在临床医生指导下进一步开展临床检查和干预；PSA<4.0ng/ml 者，每 2 年进行 1 次检测和随访，持续追踪其前列腺癌和死亡结局情况。

干预措施代码：

Intervention：

Collect venous blood for PSA screening tests in the intervention group. Biological specimens for testing should be properly preserved as required, testing should be completed within the specified time, and individuals with positive PSA results should be notified in a timely manner. It is recommended to refer them to the relevant departments for further re-examination and treatment. The screening group will conduct an initial PSA test at baseline; those with PSA >= 4.0 ng/ml will undergo further clinical examination and intervention under the guidance of a clinician. Those with PSA < 4.0 ng/ml will have a test and follow-up every 2 years, continuously tracking their prostate cancer and mortality outcomes.
