研究目的：

脓毒性休克是一种复杂的、危及生命的临床疾病，其特征是感染诱发的循环、细胞和代谢异常。占重症监护病房 (ICU) 收治脓毒症患者的 10-20%，与一般脓毒症相比，脓毒性性休克具有更高的死亡风险，在ICU内的死亡率为 37-47%，而难治性脓毒性休克的死亡率更是超过50%。目前最认可的假设是感染破坏促炎和抗炎力量的免疫平衡状态，病理性的、不受调节的炎症介质释放到循环中，这种现象也称为“细胞因子风暴”。过度炎症状态导致内皮屏障功能障碍、微循环紊乱、血管舒张、进行性组织损伤和多器官功能障碍。与传统的血液净化策略不同，细胞因子吸附治疗可通过大量去除细胞因子重建免疫稳态，然而，迄今为止，大多数证据都是描述性的或矛盾的，细胞因子吸附疗法在顽固性脓毒性休克中的应用仍在探索中。

Objectives of Study：

Septic shock is a complex, life-threatening clinical disease characterized by infection-induced circulatory, cellular, and metabolic abnormalities. Septic shock accounts for 10-20% of sepsis patients admitted to intensive care units (ICU). Septic shock has a higher risk of death than general sepsis, with a mortality rate of 37-47% in ICU and more than 50% in refractory septic shock. The most accepted hypothesis is that infection disrupts the immune balance of pro-inflammatory and anti-inflammatory forces, and pathological, unregulated inflammatory mediators are released into the circulation, a phenomenon also known as "cytokine storms." Excessive inflammatory states lead to endothelial barrier dysfunction, microcirculation disorders, vasodilation, progressive tissue damage, and multiple organ dysfunction. Unlike traditional blood purification strategies, cytokine adsorption therapy can reestablish immune homeostasis by removing large amounts of cytokines. However, to date, most evidence is descriptive or contradictory, and the application of cytokine adsorption therapy in refractory septic shock is still being explored.
