研究目的：

本中心计划进行一项多中心随机对照前瞻性临床研究，针对老年结直肠癌患者，术后使用含ω-3PUFA脂肪乳的SPN营养治疗，旨在进一步对比使用含鱼油脂肪乳剂补充性肠外营养的患者与传统肠外营养支持患者的并发症发生率，来评估前者在预防术后并发症方面的效果，以及对比两组患者术后免疫功能，炎症状态，营养状态的改善，探究含鱼油脂肪乳的SPN对老年结直肠癌患者术后恢复速度，恢复质量和安全性耐受性的影响。  

Objectives of Study：

Our center plans to conduct a multicenter, randomized, controlled, prospective clinical trial of postoperative SPN nutrition therapy containing ω-3PUFA fat emulsion in elderly patients with colorectal cancer, in order to further compare the incidence of complications between patients who use fish oil fat emulsion as supplement of parenteral nutrition and patients who use traditional parenteral nutrition support. To evaluate the effect of the former in the prevention of postoperative complications, and to compare the improvement of postoperative immune function, inflammatory state, and nutritional status between the two groups, and to explore the effect of SPN containing fish oil fat emulsion on the speed, quality, safety and tolerance of postoperative recovery in elderly patients with colorectal cancer.

纳入标准：

Inclusion criteria

排除标准：

1. 术前进行含ω-3PUFA营养支持治疗； 2. 手术前行肠外营养治疗； 3. 手术前已存在感染； 4. 肝功能异常； 5. 肾功能异常； 6. 生命体征或血流动力学参数不稳定； 7. 系统性红斑狼疮； 8. I型糖尿病； 9. 甲状腺功能亢进； 10. 已参加其他临床研究； 11. 经研究者判断，存在其他可能导致被迫中途终止研究的因素，如患有其他严重疾病（含精神疾病）需要合并治疗，酗酒，药物滥用，家庭或社会因素，可能影响到受试者安全性或依从性的因素。

Exclusion criteria：

1. Preoperative nutritional support with ω-3PUFA; 2. Preoperative parenteral nutrition; 3. Infection existed before surgery; 4. Abnormal liver function; 5. Renal dysfunction; 6. Unstable vital signs or hemodynamic parameters; 7. Systemic lupus erythematosus; 8. Type I diabetes mellitus; 9. Hyperthyroidism; 10.Have participated in other clinical research; 11. According to the investigator's judgment, there are other factors that may lead to the forced termination of the study, such as other serious diseases (including mental diseases) requiring combined treatment, alcohol abuse, drug abuse, family or social factors, and factors that may affect the safety or compliance of the subjects.

Intervention：

EN was initiated on the first postoperative day and SPN was received for at least 5 days until 80% of the energy target was achieved through EN or until discharge. EN products are routinely prescribed in hospitals and contain 1 kcal/ml of energy (14.2% protein, 31.8% lipid and 54% carbohydrate). ONS was used for enteral nutrition. The parenteral nutrition formula (with ω-3 triglycerides) includes 1.2 kcal/ml of energy and essential vitamins and minerals. Parenteral nutrition was administered via a peripheral or central vein. Oral water intake was unrestricted.

EN was initiated on the first postoperative day and SPN was received for at least 5 days until 80% of the energy target was achieved through EN or until discharge. Postoperative enteral nutrition support was the same as the experimental group. The parenteral nutrition formula was an amino acid fat emulsion with an equal calorie/nitrogen ratio and no ω-3 triglyceride, which contained essential vitamins and minerals. Parenteral nutrition was administered via a peripheral or central vein. Oral water intake was unrestricted.
