研究目的：

本研究拟通过社区冠心病患者基线调研、问卷调查和现状分析，探索在社区卫生服务站构建基于“药师工作室”的老年冠心病患者参与用药安全管理（PS-PE-MSM）的药学服务模式并进行效果评价。本研究在前期住院患者医疗过渡期药学服务研究的基础上，开展PS-PE-MSM药学服务新模式的研究，打通患者安全用药的最后一公里，实现药学服务下沉。提高患者用药依从性，减少社区老年冠心病患者药物治疗相关问题。  

Objectives of Study：

This study intends to explore and evaluate the effectiveness of a pharmaceutical service model based on a "Pharmacist Studio" for elderly patients with coronary heart disease (CHD) to participate in medication safety management (PS-PE-MSM) in community health service stations, through baseline surveys, questionnaires, and current situation analyses of community CHD patients. Building on previous research on pharmaceutical services during the medical transition period for hospitalized patients, this study aims to develop the new PS-PE-MSM pharmaceutical service model. It seeks to bridge the final gap in patients' safe medication use, bring pharmaceutical services closer to the community, enhance patients' medication adherence, and reduce medication-related issues for elderly CHD patients in the community.‌

纳入标准：

Inclusion criteria

排除标准：

第一部分 基线调研 排除标准： ①有智力障碍（Mini-cog评分＜3分）； ②不签署知情同意书 第三部分 PS-PE-MSM模式的实施与效果评价 排除标准： ①有智力障碍 （Mini-cog评分＜3分）、生活能力差（ADL量表评分＜40分）； 确诊恶性肿瘤患者。

Exclusion criteria：

Part 1 Baseline Survey ‌Exclusion Criteria‌: ‌Intellectual Impairment‌: Mini-cog score <3. ‌Non-consent‌: Refusal to sign the informed consent form. P‌art 3 Implementation and Effect Evaluation of PS-PE-MSM Model ‌Exclusion Criteria‌: ‌Impairments‌: Mini-cog score <3 or ADL score <40. ‌Malignancy‌: Diagnosed with malignant tumors. ‌
