研究利用飞秒激光技术辅助进行同种异体角膜基质透镜移植治疗圆锥角膜病人的有效性。  

Objectives of Study:

To compare the feasibility and efficacy of femtosecond laser-assisted small-incision for intrastromal implantation and penetrating keratoplasty (PKP) in keratoconus patients.

药物成份或治疗方案详述:

将供体角膜固定于人工前房中,以角膜光学区为中心,用飞秒激光辅助进行板层角膜切削,其扫描方式为同心圆螺旋式,具体参数设置:能量大小为170nJ,扫描深度为280μm,同心圆轨距4.50μm,点距4.50μm,直径7.50mm,透镜光学区直径6.50mm,过渡区为0.10mm,边切角度90°。用刀片刮除上皮后,用PRK(激光光学角膜切削术,PhotoRefractive Keratectomy)切削上层角膜基质;分离植片,并取出植片。病人术眼表面麻醉(点0.5%盐酸丙美卡因滴眼液3次)后,进行SMILE激光,切削-0.75D。SMILE激光参数:其扫描方式为同心圆螺旋式,具体参数设置:能量大小为170nJ,扫描透镜后表面同心圆轨距4.50μm,点距4.50μm,透镜边缘轨距2.00μm,点距2.00μm,角膜帽扫描同心圆轨距4.50μm,点距4.50μm,角膜帽边切口轨距2.00μm,点距2.00μm。角膜帽厚度为120μm,直径7.00mm,透镜光学区直径6.50mm,过渡区为0.10mm,最小厚度15.00μm,边切角度90°,矫正屈光度为-0.75 D球镜度,无柱镜度,角膜帽边切口位置为130°,切口角度为76°,切口弧长2.50mm;自开口处用显微镊夹出角膜基质透镜,形成角膜基质囊袋。将植片从受体SMILE切口处,用显微镊植入角膜基质囊袋内。 

Description for medicine or protocol of treatment in detail:

The donor cornea was placed into an artificial anterior chamber. A lamellar incision was made using the treatment mode of femtosecond laser-assisted lamellar keratoplasty (Carl Zeiss Meditec, Jena, Germany). The surgical parameters were described in our previous research19. The next step was myopic correction of -4.00 diopters (D) under PRK type (Wavelight GmbH, Erlangen, Germany). The lenticule was separated from the stromal bed and used as the graft. The grafts upside down surface was avoided during the implantation. The patient received topical anesthesia from proparacaine hydrochloride eye drops 3 times (Alcaine, Alcon, US). Patients underwent SMILE surgery in one randomly selected eye to achieve a myopic treatment correction of -0.75 D. The femtosecond laser parameters were described in our previous research19. The small incision (3mm) was in the 140o position. The surgeon spread the lenticule out, producing a space such as a stromal pocket inside the corneal stroma. The graft was implanted in the stromal pocket.

Inclusion criteria

排除标准:

有其他急、慢性眼病史,角膜移植术后,角膜厚度小于350μm,或者随访观察期间出现严重术后并发症、或出现怀孕、哺乳者。

Exclusion criteria:

The history of acute or chronic eye disease history, the postoperative status of corneal transplantation, the cornea thickness is less than 350 mm, or there are serious postoperative complications during the follow-up observation, or the pregnancy and lactation.
