研究目的：

研究目的：经腹会阴联合切除术（Miles）治疗直肠癌，切除了远端结肠、直肠和肛门括约肌，实施永久性结肠造口，极大地影响了患者的生活质量。1984年，Schiessel首次提出经括约肌间切除术（intersphincteric resrction，ISR）治疗低位直肠癌，实施结肠肛管吻合，保留了肛门，避免了永久性造口。由于ISR能将直肠的切除范围扩展到距齿状线下方1 cm，最大限度保留括约肌，明显提高了患者的生活质量，目前，对于超低位直肠癌患者ISR已经被广泛应用。但Miles与ISR两种术式术后的长期疗效哪种更有利于患者，相关的对照研究不多，也缺乏随机对照试验。本研究采取回顾性分析方法，比较ISR与Miles治疗低位直肠癌的近期、长期效果。 由于在随机对照试验中，在必须保证患者安全和伦理约束的情况下，对患者实行是否挖出肛门进行随机对照是不切实际的，是无法执行的，因此到目前为止仍没有这方面的前瞻性随机对照。因此，我们欲在本中心开展的一项单中心回顾性研究，对比两组预后。  

Objectives of Study：

Aim of the study:Transperineal perineal colectomy (APR) for rectal cancer removes the distal colon, rectum, and anal sphincter, and implements a permanent colostomy, which greatly affects the patient's quality of life.In 1984, Schiessel firstly proposed the transsphenoidal intersphenoidal resorption (ISR) for the treatment of low-grade rectal cancer by performing colorectal-anal anastomosis, which preserves the anus and avoids permanent stoma. Because ISR can extend the resection of rectum to 1 cm below the dentate line, maximally preserving the sphincter, and significantly improving the quality of life of patients, ISR has been widely used for patients with ultra-low rectal cancer. However, there are not many relevant controlled studies on which of the two postoperative long-term outcomes of Miles and ISR is more beneficial to patients, and there is a lack of randomised controlled trials. In this study, a retrospective analysis was conducted to compare the immediate and long-term outcomes of ISR and Miles for low rectal cancer. As it is impractical and unenforceable to implement a randomised controlled trial with or without anal excavation in patients where patient safety and ethical constraints must be ensured, there are no prospective randomised controls in this area to date. Therefore, we would like to conduct a large retrospective study in our centre comparing the prognosis of the two groups, and at a later stage, depending on the cases, to conduct a retrospective study in conjunction with several other centres.

开放性手术、术前合并远处转移的姑息性手术、术前合并急性肠梗阻或肠穿孔的手术，以及其他类型的无吻合手术，包括哈特曼手术和经肛门局部切除术。

Exclusion criteria：

Distant metastases, open surgery, palliative surgery with preoperative combination of distant metastases, surgery with preoperative combination of acute intestinal obstruction or intestinal perforation, and other types of anastomosis-free surgery, including Hartmann's procedure and transanal local excision.
