研究目的:

冠状动脉微血管病变(CMVD)是冠状前小动脉和小动脉的结构和(或)功能异常所致的劳力性心绞痛或心肌缺血客观证据的临床综合征。多项研究发现CMVD患者发生心血管事件风险提高20%,12年内心源性死亡风险增加3.3倍,且常规治疗效果不佳。早期准确诊断对CMVD预后及治疗具有重要价值。PET通过测定心肌血流量和冠脉血流储备来反映冠脉微血管功能。但受限于空间分辨率,临床PET不能准确地量化血管扩张剂引起的心外膜下至心内膜下的灌注梯度。心脏磁共振可以对MBF和心肌灌注储备进行定量检测,但当比较区域MBF时,相关性不如PET。PET和 MRI的图像处理过程复杂,且缺少获取灌注信息的心脏成像定量分析软件。本项目拟用超高分辨率全数字PET/MRI探索PET、MRI双灌注成像在CMVD 评估中作用;同时基于PnI-Rayplus-RWE科研平台,结合深度学习,开发核医学心脏图像定量智能分析方法。建立智能诊断CMVD的新诊断和预后模式,并通过临床病例进行有效验证,探索CMVD的诊断新路径、新方法和新思路。  

Objectives of Study:

Coronary microvascular disease (CMVD) is a clinical syndrome with objective evidence of labor-induced angina pectoris or myocardial ischemia caused by structural and/or functional abnormalities of anterior coronary arteries and arterioles. Multiple studies have found that patients with CMVD have a 20% increased risk of cardiovascular events, a 3.3-fold increase in the risk of cardiogenic death within 12 years, and the conventional treatment effect is not good. Early and accurate diagnosis has important value for the prognosis and treatment of CMVD. PET reflects coronary microvascular function by measuring myocardial blood flow and coronary blood flow reserve. However, limited by spatial resolution, clinical PET cannot accurately quantify the subepicardial to subendocardial perfusion layer caused by vasodilators. Cardiac magnetic resonance can quantitatively detect MBF and myocardial perfusion reserve, but when comparing regional MBF, the correlation is not as good as PET. The image processing process of PET and MRI is complicated, and there is a lack of quantitative analysis software for cardiac imaging to obtain perfusion information. This project intends to use ultra-high-resolution all-digital PET/MRI to explore the role of PET and MRI double perfusion imaging in CMVD assessment; at the same time, based on the PnI-Rayplus-RWE scientific research platform, combined with deep learning, develop a quantitative intelligent analysis method for nuclear medicine cardiac images. Establish a new diagnosis and prognosis model for intelligent diagnosis of CMVD, and effectively verify it through clinical cases to explore new pathways, new methods and new ideas for CMVD diagnosis.

Inclusion criteria

排除标准:

①对双嘧达莫、腺苷等显影剂过敏; ②收缩压<90 mmHg(1 mmHg=0.133 kpa)、心率<40 次/min; ③阻塞性冠心病、冠脉痉挛、急性心肌炎、心包炎、心肌病、心肌桥、近期急性心肌梗死等原因导致的心绞痛; ④急性全身系统疾病或感染、严重的肝肾功能障碍、未控制的代谢性疾病; ⑤妊娠期及哺乳期女性; ⑥哮喘或持续性喘息者。

Exclusion criteria:

① Allergic to developers such as dipyridamole and adenosine; ② Systolic blood pressure < 90 mmHg (1 mmHg = 0.133 kpa), heart rate < 40 beats/min; ③ Angina caused by obstructive coronary heart disease, coronary spasm, acute myocarditis, pericarditis, cardiomyopathy, myocardial bridge, and recent acute myocardial infarction; ④ Acute systemic diseases or infections, severe liver and kidney dysfunction, and uncontrolled metabolic diseases; ⑤ Pregnant and lactating women; (6) Asthma or persistent wheezing.
