研究目的：

随着人口老龄化问题的日益突出，需要手术治疗的老年患者也逐渐增多，盆底器官重建术是老年女性常见的手术类型。术后谵妄(Postoperative Delirium, POD) 是老年患者术后常见的严重并发症之一，可致住院时间延长、医疗消耗增加、预后较差，增加家庭与社会负担，目前缺乏有效的治疗手段。如何预防 POD 的发生， 优化围术期脑健康管理已经成为需要迫切解决的临床课题。围术期阿片类药物的使用是 POD 常见的危险因素，可以通过低阿片麻醉技术减少其使用量，可能会降低 POD 的发生率。本研究拟通过脑电监测下低阿片麻醉技术，在保证患者充分镇痛的基础上，减少围术期阿片药物消耗量，探索预防 POD 发生的新的有效干预措施，以达到预防和治疗老年患者脑功能障碍、维护围术期脑健康的目的。  

Objectives of Study：

With the aging of the population, the number of elderly patients who need surgical treatment is increasing. Pelvic organ reconstruction is a common type of surgery for elderly women. Postoperative Delirium (POD) is one of the common serious complications in elderly patients after surgery, which can lead to prolonged hospital stay, increased medical consumption, poor prognosis, and increased family and social burden. At present, there is a lack of effective treatment methods. How to prevent the occurrence of POD and optimize perioperative brain health management have become clinical issues that need to be urgently solved. Perioperative opioid use is a common risk factor for POD, which can be reduced by low-opioid anesthesia techniques, which may reduce the incidence of POD. This study intends to explore new effective intervention measures to prevent the occurrence of POD by using low-opioid anesthesia technique under electroencephalogram (EEG) monitoring to ensure adequate analgesia and reduce perioperative opioid consumption, so as to achieve the purpose of preventing and treating brain dysfunction in elderly patients and maintaining perioperative brain health.

纳入标准：

Inclusion criteria

排除标准：

(1) 既往存在精神类疾病(如帕金森氏病、抑郁症、焦虑症等)；(2) 全身麻醉禁忌症或患者拒绝实施全身麻醉；(3)已知研究药物其成分或其他本研究用药过敏者；(4) 一周内使用阿片类药物及其他镇静镇痛药物；(5) 有药物滥用史者；(6)有严重心、肺、肝、肾功能障碍者。

Exclusion criteria：

(1) previous psychiatric diseases (such as Parkinson's disease, depression, anxiety, etc.); (2) contraindications to general anesthesia or patients refusing general anesthesia; (3) patients with known ingredients of the study drug or allergy to other drugs used in the study; (4) use of opioids and other sedative and analgesic drugs within one week; (5) patients with a history of drug abuse; (6) patients with severe heart, lung, liver and kidney dysfunction.

麻醉维持采用瑞芬太尼(0.1 μg/kg/min)靶控泵注，复合右美托咪定(0.3 μg/kg)静脉滴注，术中通过 IoC 意识 指数调节患者镇痛及镇静水平，术后镇痛应用阴部神经阻滞的无阿片方案。

使用 SPSS软件生成120个随机数字，平均分为两组，确定每个随机数字的分组，制作随机数字卡，将随机数字卡和相应的干预措施代码(1代表对照组， 2代表低阿片组)放入不透光的密封信封中。受试者按就诊顺序抽取信封进行分组，按 1∶1 的比例分为观察组和对照组，每组60例。

Using SPSS software, 120 random numbers were generated and divided equally into two groups, the grouping of each random number was determined, random number cards were made, and the random number cards and corresponding intervention codes (1 for control group and 2 for low-opioid group) were placed in light-tight sealed envelopes. The patients were divided into the observation group and the control group according to the ratio of 1∶1, with 60 cases in each group.
