研究目的:

(1)识别多病共存患者疾病进展多维风险因素,构建疾病进展风险分层评估模型,为多病共存患者分层干预方案制定提供理论依据。 (2)探究多病共存患者疾病进展机理,构建多病共存患者分层干预方案,据此研发多病共存患者智能动态适配系统,并进行系统安全度量和灵敏度测量。 (3)探究多病共存患者智能动态适配系统的应用效果和经济学价值,优化基于多元主体的多病共存患者健康管理政策,为政府部门出台相关健康管理政策提供参考依据。  

Objectives of Study:

(1) To identify multidimensional risk factors for disease progression in patients with multiple diseases, construct a stratified risk assessment model for disease progression, and provide theoretical basis for formulating stratified intervention plans for patients with multiple diseases. (2) To explore the disease progression mechanism of patients with multiple diseases, construct a hierarchical intervention scheme for patients with multiple diseases, develop an intelligent dynamic adaptation system for patients with multiple diseases, and measure system safety and sensitivity. (3) Explore the application effect and economic value of the intelligent dynamic adaptation system for patients with multiple diseases, optimize health management policies for patients with multiple diseases based on multiple agents, and provide reference for government departments to introduce relevant health management policies.

Inclusion criteria

排除标准:

1.本研究开始前4周内曾参加过其他试验;
2.存在精神异常;
3.患有肿瘤,且近半年接受过化疗或放疗;
4.患有心肌梗死;
5.合并其他重大疾病,预期自然病程小于1年;
6.居家无法访问网络;
7.正处于孕期和哺乳期;

Exclusion criteria:

1.Other trials were conducted within 4 weeks prior to the start of the study;
2.Have a mental disorder;
3.Have a tumor and have received chemotherapy or radiation therapy in the last six months;
4.Had a heart attack;
5.Combined with other major diseases, the expected natural course of disease is less than 1 year;
6.Internet access is not possible at home;
7.Internet access is not possible at home;
