研究目的：

1) 全面刻画脆弱老年群体相较于非脆弱群体的健康特征，探究与脆弱性相关的关联因素。 2) 开发构建一套适用于脆弱老年人群的综合照护干预和实施策略。 3) 评估综合照护策略对脆弱老年人群的生存质量和健康状况的影响。  

Objectives of Study：

1. To comprehensively profile the health characteristics of vulnerable elderly populations compared to non-vulnerable groups and explore the associated factors related to vulnerability. 2. To develop and construct an integrated care intervention and implementation strategy tailored for the vulnerable elderly population. 3. To evaluate the impact of the integrated care strategy on the quality of life and health status of the vulnerable elderly population.

排除标准：

-年龄60岁以下； -罹患严重慢性疾病且期望寿命不足3年。

Exclusion criteria：

-Individuals under the age of 60; -Individuals who suffer from severe chronic diseases with a life expectancy of less than three years.
