本研究是为了探讨有效且快速的慢阻肺临床信息的收集及管理方案，并建立具有搜集、储存、管理、随访及分析等强大功能的数据平台。  

Objectives of Study：

The purpose of this study is to explore an effective and rapid way to collect and manage COPD clinical information and to establish a data platform with powerful functions such as gathering, storage, management, follow-up and analysis.

纳入标准：

Inclusion criteria

排除标准：

1、研究者判断患者不能理解研究过程；
2、患者不能/不愿回答问卷、提供病历、检查报告等相关资料；
2、患者拒绝参与慢阻肺管理，包括慢阻肺登记、随访等；

Exclusion criteria：

1. Researchers determin that the patients can not understand the research process;
2. Patients can not/are unwilling to answer the questionnaires, provide patient records, provide inspection reports and other relevant information;
2. Patients refuse to participate in the management of COPD, including COPD registration and follow-up,etc.
