研究目的:

本研究通过近红外脑功能成像系统和Bertec 4D平衡系统,希望探究足底刺激对CAI人群姿势控制和感觉统合及其相关皮层活动的即刻影响。为完善慢性踝关节不稳后中枢机制理论体系提供脑区激活证据支持,为临床采用足底感觉刺激疗法提高CAI者姿势控制能力提供实践证据支持,促进更加精准的CAI康复方案的制定。  

Objectives of Study:

In this study,functional near- infrared spectroscopy(fnirs) and Bertec 4D balance system were used to investigate the immediate effects of plantar stimulation on postural control, sensory integration and their related cortical activities. In order to improve the theoretical system of central mechanism after chronic ankle instability, to provide evidence support for brain region activation, to provide practical evidence support for the clinical use of plantar sensory stimulation therapy to improve the postural control ability of CAI patients, and to promote the formulation of more accurate CAI rehabilitation programs.

Inclusion criteria

排除标准:

1).最近3个月内发生下肢急性损伤或头部受伤; 2).有下肢手术或骨折史等可能影响踝关节运动或平衡的问题 3).存在已知的平衡障碍或前庭障碍,和/或其他神经系统、前庭系统、认知缺陷、脑震荡等疾病; 4).正在服用可能影响认知或平衡的药物(麻醉、抗抑郁/焦虑药);

Exclusion criteria:

1). Have experienced an acute lower limb injury or head injury within the past 3 months. 2). Are currently taking medications that may affect cognition or balance (such as anesthesia, antidepressants/anxiolytics). 3). Have a history of lower limb surgery or fracture, or other potential issues that may affect ankle movement or balance. 4). Have known balance disorders or vestibular disorders, and/or other neurological, vestibular, cognitive deficits, concussions, or other related conditions.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

数据收集:将一般信息(身高、体重、BMI、年龄、扭伤史、踝关节稳定性)、坎伯兰踝关节指数评分(CAIT)制作成问卷,通过问卷星小程序收集;对每位受试者进行编号,用便携式近红外设备记录脑激活实验数据,记录基线20s和任务状态20s叠加3次,以符合相关数据分析的要求,每个文件的数据为(基线-任务状态-基线)×3,即至少保留2min20s的数据记录。单腿站立数据由测力台设备记录,分别记录睁眼和闭眼状态下的双下肢单腿站立的 COG 数据,同时测量单腿站立状态下的脑区激活程度,并记录在配备有近红外装置的计算机中。感觉统合数据由配备有 Bertec 设备的计算机收集,分别生成六种感觉条件下的平衡评分和综合评分。VAS 评分由受试者在研究过程中根据自己的感觉进行评估,并由研究人员即时记录。 数据管理:研究人员的个人电脑设备。在电脑上建立研究文件夹,为不同的研究指标分别建立子文件夹,包括SOT子文件夹、COG摆动速度子文件夹、fNIRs子文件夹、VAS子文件夹和一般信息子文件夹,并在每个子文件夹下对不同组(CAI组、健康组)的数据进行分类管理。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

Data collection: general information (height, weight, BMI, age, history of sprains, ankle stability), Cumberland Ankle Index Score (CAIT) are made into a questionnaire, which will be collected by the questionnaire star applet; each subject will be numbered, and the experimental data of brain activation will be recorded by the portable NIRX device, recording baseline 20s and task state 20s superimposed three times in order to comply with the requirements of the relevant data analysis.The data for each file contact (baseline - task state - baseline) x 3 so at least 2min20s of data recording will be kept.Single-leg stance data will be recorded by the force plate device, which records the COG data of single-leg station for both lower limbs under eye-open and eye-closed conditions respectively, and the degree of activation of the brain regions will be measured simultaneously in the single-leg station condition, which will be recorded in the computer equipped with the fNIRs device.Sensory integration data will be collected by a computer equipped with the Bertec device, generating equilibration scores and composite scores for each of the six sensory conditions.VAS scores will be assessed by the subjects according to their own perceptions during the course of the study, and will be recorded instantly by the researcher. Data management: personal computer equipment of the researcher.Study folders will be set up on the computer, with separate subfolders for different study metrics, including SOT subfolder, COG swing speed subfolder, fNIRS subfolder, VAS subfolder, and general information subfolder, and data will be categorised and managed under each subfolder for the different groups (CAI group, health group)
