研究目的：

(1)基于“中枢-外周-中枢”的闭环康复理论指导，运用外周MNS结合中枢tDCS治疗脑损伤后pDOC，评估治疗前、治疗20次后CRS-R量表评分，探究“中枢-外周-中枢”的闭环治疗模式是否优于单纯外周或中枢刺激治疗； (2)基于脑干听觉诱发电位各波潜伏期、事件相关电位MMN、P300的潜伏期的变化，脑电分析的静息态脑网络链接变化，探究“中枢-外周-中枢”闭环康复理论对pDOC治疗的中枢重塑机制。  

Objectives of Study：

(1) Based on the theoretical guidance of "central-peripheral-central" closed-loop rehabilitation, peripheral MNS combined with central tDCS was used to treat pDOC after brain injury, and the CRS-R scale score was evaluated before treatment and after 20 treatments, so as to explore whether the closed-loop treatment mode of "central-peripheral-central" was better than simple peripheral or central stimulation therapy. (2) Based on the changes in the latency of each wave of brainstem auditory evoked potentials, the incubation period of event-related potentials MMN and P300, and the changes of resting-state brain network links from EEG analysis, the central remodeling mechanism of "central-peripheral-central" closed-loop rehabilitation theory on pDOC treatment was explored.

纳入标准：

Inclusion criteria

排除标准：

①存在呼吸、循环等系统的严重并发症，生命体征不平稳者； ②合并有癫痫持续状态者； ③存在颅内感染者； ④年龄大于70岁或年龄小于18岁； ⑤CT或MRI示脑水肿明显； ⑥家属或其监护人不愿或不能配合治疗，拒绝填写《知情同意书》； ⑦病情不稳定和其他患严重心、肺、肝、肾疾病者。

Exclusion criteria：

(1) Those with serious complications of respiratory, circulatory and other systems, and unstable vital signs; (2) Patients with status epilepticus; (3) Patients with intracranial infection; (4) Age greater than 70 years old or less than 18 years old; (5) CT or MRI showed obvious cerebral edema; (6) Family members or their guardians are unwilling or unable to cooperate with the treatment and refuse to fill in the Informed Consent Form; (7) Those who are unstable and suffer from other serious heart, lung, liver and kidney diseases.

tDCS阳极置于优势半球（左侧）的前额叶背外侧区(根据国际10-20电极系统的定位方法，左侧大脑前额叶背外侧位于F3，操作时将tDCS 阳极电极片的中心放置于此区) ，阴极置于对侧肩部; 电极片大小: 6.5cm×6.5cm。刺激强度为2mA，每天一次，每次20分钟，每天1次，10次为一个疗程，共治疗2个疗程。

The anode of tDCS is placed over the dorsolateral prefrontal cortex of the dominant hemisphere (the left side), specifically at the location F3 according to the international 10-20 electrode placement system (the center of the anode electrode pad is positioned over this area during the procedure). The cathode is placed on the contralateral shoulder. The size of the electrode pads is 6.5cm × 6.5cm. The stimulation intensity is set at 2mA, with one session lasting 20 minutes, administered once a day. Ten sessions constitute one treatment course, and a total of two treatment courses are given.

One patch of the median nerve electrical stimulation device is placed on the median nerve, located 2 cm above the transverse palmar crease of the patient's right wrist. The other patch is placed approximately 4 cm above the transverse wrist crease, ensuring that the distance between the two electrode patches is over 1 cm. The output stimulation waveform is a unilateral square wave with a frequency of 40 Hz, a pulse width of 300 microseconds (μs), and a stimulation intensity generally ranging from 10 to 20 mA, based on the observed flexion and twitching of the right middle finger. The mode is set to the median nerve stimulation mode, with a treatment duration of 2 hours per day. Ten days constitute one treatment course, and a total of two treatment courses are administered

The collection of clinical data in the CRF table was primarily responsible by Dr. Wang Jiasheng from the research team, including the subjects' general information and CRS-R scale scores before and after intervention. The resting-state electroencephalogram (EEG), brainstem auditory evoked potentials (BAEP), and event-related potential (ERP) examinations were conducted by Dr. Xue Hui from the Neuroelectrophysiology Laboratory.
