研究目的：

本研究旨在通过针对不同严重程度的COPD 患者采用个体化的呼吸生理治疗、肌肉训练等肺康复干预手段，进一步深入研究肺康复治疗是否能降低COPD患者急性发作频率、再入院率及死亡率，并探究更好的肺康复综合治疗方案。

Objectives of Study：

In this study, we are going to conduct a pulmonary rehabilitation programme that included breathing retraining, exercise training and other pulmonary rehabilitation interventions on patients with different severity of COPD and to further investigate whether it can reduce the frequency of acute seizures, readmission rates and mortality in patients with COPD. And explore better integrated rehabilitation programs for pulmonary rehabilitation.

药物成份或治疗方案详述：

(1) 缩唇联合腹式呼吸锻炼，(2) 行走或踏车，(3) 全身性呼吸体操锻炼: 包括扩胸、弯腰、下蹲等动作结合在一起。按照由慢到快，由易到难，循序渐进的原则进行( 锻炼期间测定患者指脉氧饱和度，必要时可给与吸氧以保证SPO2 ＞ 90%) ，锻炼前有专职医护人员演示指导，掌握以上规则后进行家庭训练。每周3 次，每次30 min。

Description for medicine or protocol of treatment in detail：

(1)pursed-lip and abdominal breathing training; (2) walking or cycling;(3) systemic breathing exercises include: chest expansion, bending, squatting and other exercises. In accordance with the principle of slow to fast, easy to difficult and step by step(oxygen saturation was measured continuously during the walk, if necessary, supplemental oxygen was permitted to ensure SPO2> 90%), before the training professional medical staff will demonstrate and guide, then subjects will conduct the family training. 3 times a week for 30 minutes each time.

纳入标准：

Inclusion criteria

排除标准：

目前合并有其他严重的心肺疾病的，如哮喘、支气管扩张、肺癌、不稳定性心脏疾病等患者，（2）处于慢性阻塞性肺疾病、肺间质纤维化的急性加重期，如合并肺部感染，严重的呼吸衰竭等，（3）合并其他肌肉神经性功能缺陷，（4）不能完成肺康复训练及6 分钟步行试验、肺功能检查的，（5）过去12 个月参加过肺康复训练的。

Exclusion criteria：

(1) Patients with other serious cardiopulmonary disease including asthma, bronchiectasis, lung cancer, unstable cardiac disease;
(2) Patients in state of acute exacerbations of chronic obstructive pulmonary disease or interstitial pulmonary fibrosis that associate pulmonary infection, severe respiratory failure;
(3) Patients combined with other muscle neurological deficiencies;
(4) Patients unable to complete a pulmonary rehabilitation program, 6MWD and pulmonary function test;
(5) Patients previous participated in a pulmonary rehabilitation program.
