研究目的：

研究奥赛利定对围术期神经认知障碍（perioperative neurocognitive disorders，PND）的影响，并通过基础研究解释其中机制，同时观察奥赛利定镇痛的有效性及副反应发生情况，为老年人围术期镇痛管理提供更优的解决方案，同时也可开拓PND防治新路线。

Objectives of Study：

The study aims to investigate the effects of dexmedetomidine on perioperative neurocognitive disorders (PND) and elucidate the underlying mechanisms through basic research. Simultaneously, it observes the efficacy of dexmedetomidine as an analgesic and the occurrence of side effects, aiming to provide better pain management solutions for elderly patients during the perioperative period. Additionally, the research may open up new avenues for the prevention and treatment of PND.

纳入标准：

Inclusion criteria

排除标准：

(1)患者或家属拒绝参加本研究；(2)患有心血管、呼吸、肝、肾、中枢神经系统等严重疾病；(3)预期寿命不足12个月；(4) MMSE（简易精神状态量表）评分低于23分的；(5) 目前正在使用镇静剂、抗抑郁药或皮质类固醇；(6)酗酒、吸毒；(7)跟踪困难或者依从性差的；

Exclusion criteria：

(1) Patient or family refusal to participate in this study. (2) Severe diseases affecting cardiovascular, respiratory, hepatic, renal, or central nervous systems. (3) Life expectancy less than 12 months. (4) MMSE (Mini-Mental State Examination) score less than 23. (5) Currently using sedatives, antidepressants, or corticosteroids. (6) Alcohol or drug abuse. (7) Difficulty in follow-up or poor compliance.

Blinding：	After the patient awakens from anesthesia, a researcher who is unaware of the randomization procedure opens the envelope and assigns the patient to either the oliceridine group or the morphine group. The postoperative follow-up personnel are also blinded to the group assignments.
