探讨二元疾病管理理论在头颈癌患者及照顾者出院准备方案构建中的应用，为提高头颈癌患者及照顾者出院准备度，减轻照顾者照顾压力提供临床证据  

Objectives of Study：

Discussing the application of binary disease management theory in the construction of discharge preparation plans for head and neck cancer patients and caregivers, providing clinical evidence to improve discharge preparations for head and neck cancer patients and caregivers and reduce care pressure

排除标准：

第一部分： 德尔菲专家函询法专家排除标准： 1. 无法完成函询的专家； 2. 在第一轮函询中对所有条目表示不熟悉的专家。 第二部分：基于二元疾病管理理论的出院准备服务方案对头颈癌患者术后出院准备度干预效果的随机对照研究 患者排除标准: 1. 存在沟通障碍（气切或气管造瘘患者除外）或精神疾病; 2. 存在严重脏器损害; 3. 不知晓病情。 照顾者排除标准： 存在沟通障碍或精神疾病

Exclusion criteria：

Part One: Expert Exclusion Criteria for Delphi Expert Consultation Method 1. Experts unable to complete the consultation rounds; 2. Experts indicating unfamiliarity with all items in the first-round consultation. Part Two: Randomized Controlled Study on the Intervention Effect of a Discharge Preparation Service Program Based on Dual-Disease Management Theory on Post-Discharge Readiness for Head and Neck Cancer Patients After Surgery Patient Exclusion Criteria: 1. Presence of communication difficulties or mental disorders; 2. Presence of severe organ damage; 3. Unaware of their diagnosis. Caregiver Exclusion Criteria: Presence of communication difficulties or mental disorders.

专家函询表以纸质或电子邮件的方式发送，第 1 轮问卷收集完成后，对函询结果进行整理、分析和讨论，对干预方案进行相应增减和修改，专家意见较为一致，结束函询。条目删除的标准：当某项指标有 70%以上的专家建议删除 和（或）指标重要性赋值均数＜3.5 分，变异系数>=0.25 时即予以删除，其他专家修改意见经课题组讨论作出最终决定。

TThe expert consultation form was sent by paper or e-mail. After the first round of questionnaire collection was completed, the results of the consultation were collated, analyzed and discussed, and the intervention plan was correspondingly increased, decreased and modified. The expert opinions were more consistent and the consultation was concluded. The criteria for item deletion : when more than 70 % of experts suggest deleting an indicator and ( or ) the mean value of the importance assignment of the indicator is less than 3.5 points, and the coefficient of variation is greater than or equal to 0.25, it is deleted, and other expert amendments are discussed by the research group to make a final decision.
