研究目的：

1.1 建立基于物联网技术COPD肺康复管理平台，开展规范化、全程化和智能化肺康复全流程管理，实现医疗单位双向转诊、远程咨询和家庭肺康复； 1.2 推动肺康复分级诊疗在社区基层医院落地实施，改善肺康复医疗资源的合理分配，加强与社区基层合作，实现COPD患者肺康复全程管理； 1.3 搭建“医院-社区-家庭” 三位一体管理模式，实现医疗资源优化配置，解决患者肺康复依从性差、长期随访和远程治疗难以实现等问题，探讨该模式对稳定期COPD患者肺康复管理的有效性，评价应用于基层COPD规范化管理路径的可行性。

Objectives of Study：

1.1 Establish a COPD pulmonary rehabilitation management platform based on IoT technology, carry out standardized, comprehensive, and intelligent management of the entire process of pulmonary rehabilitation, and achieve two-way referral, remote consultation, and home pulmonary rehabilitation in medical units; 1.2 Promote the implementation of graded diagnosis and treatment for pulmonary rehabilitation in community grassroots hospitals, improve the rational allocation of pulmonary rehabilitation medical resources, strengthen cooperation with community grassroots, and achieve full process management of pulmonary rehabilitation for COPD patients; 1.3 Establish a three in one management model of "hospital community family" to optimize the allocation of medical resources, solve problems such as poor compliance of patients with pulmonary rehabilitation, difficulty in achieving long-term follow-up and remote treatment, explore the effectiveness of this model for pulmonary rehabilitation management of stable COPD patients, and evaluate the feasibility of applying it to the standardized management path of COPD at the grassroots level.

Inclusion criteria

排除标准：

（1）合并慢阻肺以外的重要疾病；（2）实验室检查有显著临床意义异常的患者；（3）合并有临床意义的尘肺等限制性通气功能障碍疾病；（4）合并未治愈或进展期的恶性肿瘤；（5）合并活动性肺结核；（6）合并有危及生命的肺栓塞，合并有临床意义的 α1-抗胰蛋白酶缺乏症或囊性纤维化的患者；（7）有肺叶切除术史；（8）下肢活动障碍，无法独自完成六分钟步行试验评估；（9）符合肺功能检查绝对禁忌症；（10）筛选前3个月内参加过其他呼吸系统临床试验；（11）研究者认为不适于参加临床研究的患者。

Exclusion criteria：

(1) Merge important diseases other than chronic obstructive pulmonary disease; (2) Patients with significant clinical abnormalities in laboratory tests; (3) Merge clinically significant restrictive ventilation dysfunction diseases such as pneumoconiosis; (4) Merge untreated or advanced malignant tumors; (5) Merge active pulmonary tuberculosis; (6) Patients with life-threatening pulmonary embolism, clinically significant alpha 1-antitrypsin deficiency, or cystic fibrosis; (7) History of lobectomy; (8) Lower limb mobility impairment, unable to complete the six minute walking test assessment alone; (9) Complies with absolute contraindications for pulmonary function tests; (10) Have participated in other respiratory clinical trials within the past 3 months prior to screening; (11) Researchers believe that patients who are not suitable to participate in clinical studies.
