1.探索构建“3+2+1”医疗照护过渡期的连续性药学服务模式； 2.探索基层药师慢病药学服务临床路径，为药学联合体的建立提供实践经验； 3.实现药学服务下沉，提升社区药师慢病管理能力。

Objectives of Study：

1. Explore a "3+2+1" continuous pharmaceutical care model during care transitions. 2. Explore the clinical pathway of chronic disease pharmaceutical services provided by grassroots pharmacists, and provide practical experience for the establishment of pharmaceutical consortia. 3. Achieve the sinking of pharmaceutical services and enhance the chronic disease management capabilities of community pharmacists.

Inclusion criteria

排除标准：

①合并严重肝、肾功能异常患者；②恶性肿瘤患者；③意识、精神和交流障碍者；④未能签署知情同意者。

Exclusion criteria：

1 Patients combined with severe hepatic and renal function abnormalities. 2 Patients with malignant tumors. 3 People with consciousness, mental and communication disorders. 4 People who failed to sign the informed content.

由非研究者采用Excel 2017软件生成包含60个数字的随机数字表，并将写有60个随机数字的纸条装入不透明信封，按照随机数字表的顺序在信封上标注序号。由研究者对符合纳入和排除标准的患者按住院顺序依次拆开信封，根据信封内的数字分为干预组（奇数）和对照组（偶数）

Randomization Procedure (please state who generates the random number sequence and by what method)：

The non-researcher used Excel 2017 software to generate a random number table containing 60 numbers, and put a paper strip with 60 random numbers into an opaque envelope, and marked the serial number on the envelope in the order of the random number table. The investigator opened the envelopes for patients who met the inclusion and exclusion criteria in the order of hospitalization, and divided them into intervention group (odd number) and control group (even number) according to the number in the envelope.
