研究目的：

基于个人和家庭自我管理理论的过程维度形成双相障碍青少年非自杀性自伤患者的出院计划，并进行临床实践，提高双相障碍青少年非自杀性自伤患者的出院准备度水平和自我管理水平，增强患者的自我效能以及改善患者生活的质量。  

Objectives of Study：

To form a discharge plan for adolescent non-suicidal self-injury patients with bipolar disorder based on individual and family self-management theories and to conduct clinical practice to improve the level of readiness for discharge and self-management of adolescent non-suicidal self-injury patients with bipolar disorder, to enhance the self-efficacy of the patients as well as to improve the quality of life of the patients.

纳入标准：

Inclusion criteria

排除标准：

①伴严重精神症状，如幻觉、妄想； ②伴精神发育迟滞； ③合并其他严重躯体疾病； ④中途出院； ⑤不愿配合者。

Exclusion criteria：

① With severe mental symptoms, such as hallucinations and delusions; ② with mental retardation; ③ Combined with other serious physical diseases; ④ Discharged from the hospital in the middle of the day; ⑤ Those who are unwilling to cooperate.

研究对象在入院后第一天开始参加双相障碍、自伤及自伤自我管理相关知识的课程，进入科室微信管理群；出院前1天与护理人员共同制定出院计划，以促进出院后的衔接；出院后研究者根据在院内和患者制定的院外出院计划，及时向微信群内推送相关内容以及组织有关的集体活动。出院后3个月内进行随访，出院后每周1次在群中推送自伤及自我管理的知识，采用PPT的形式进行推送；每周1次在群内组织一次集体活动，每2周对患者进行一次电话随访，护士会重点关注患者的不良情绪，及时感知、疏导患者的不良情绪；研究者提醒患者每月1次按时复诊。（见研究计划书中双相障碍青少年非自杀性自伤患者的出院计划终稿）。

干预措施代码：

Intervention：

The participants start to attend the course on knowledge related to bipolar disorder, self-injury and self-injury self-management on the first day after admission, and join WeChat management group; 1 day before discharge, they work with nursing staff to develop a discharge plan to facilitate post-discharge articulation; after discharge, the researcher push the relevant content as well as organized relevant group activities by the WeChat, based on the out-of-hospital discharge plan developed by the researcher and the patients. Follow-up visits will be conducted within 3 months after discharge; knowledge of self-injury and self-management will be pushed to the group once a week after discharge, using the form of PPT; collective activities will be organized in the group once a week, and telephone follow-up every two weeks; nurses would focus on the patients' adverse emotions, and perceive and channel them in a timely manner; and the researcher will remind the patients to follow up their medical appointments once a month in a timely manner. 主要是常规进行治疗、护理和疾病健康教育。患者入院第一天由责任护士进行入院宣教和风险评估，住院一周进行常规疾病相关健康教育，出院前1天进行出院宣教。出院后由出院计划小组以外的护士随访，研究者每月对患者进行一次电话随访，随访内容主要为患者休息与活动、饮食、用药、告知定期复查等。

Intervention：

The main components are routine treatment, nursing care and disease-related health education. Patients will be admitted to the hospital on the first day for admission and risk assessment by the nurse in charge, hospitalized for one week for routine disease-related health education, and discharged 1 day before discharge. After discharge, patients will be followed up by nurses other than the discharge planning team, and telephone followe-up will be conducted once a month, focusing on the patients' rest activities, diet, medication, and advice on regular review.
