研究目的：

（1）评估达格列净对原发性IgA肾病患者尿蛋白、肾功能、血压的影响；（2）观察达格列净对原发性IgA肾病患者血浆白蛋白、血脂、血尿酸、电解质、血红蛋白等生化指标及免疫球蛋白IgA、IgG、IgA/C3比值等的影响；（3）探究达格列净对原发性IgA肾病患者肾功能恶化、发生终末期肾病、心血管或肾病死亡、全因死亡风险的影响，评估达格列净治疗原发性IgA肾病患者的安全性。

Objectives of Study：

1.Evaluate the effects of dapagliflozin on urinary protein, renal function and blood pressure in patients with primary IgA nephropathy; 2.Observe the effects of dapagliflozin on plasma albumin, blood lipid, uric acid, electrolyte, hemoglobin and immunoglobulin IgA, IgG, IgA/C3 ratio in patients with primary IgA nephropathy 3.Explore the effects of dapagliflozin on renal deterioration, end-stage renal disease, cardiovascular or renal death, and all-cause death in patients with primary IgA nephropathy, and to evaluate the safety of dapagliflozin in the treatment of patients with primary IgA nephropathy.

药物成份或治疗方案详述：

治疗组予达格列净10 mg/d，对照组给予同等剂量安慰剂。其它治疗相同，两组患者均进行个体化支持治疗，尽量保持两组患者之间辅助用药差别不大，并根据患者具体情况进行相应调整，保持血压、血糖、血脂等的相对稳定。

Description for medicine or protocol of treatment in detail：

The treatment group was given dapagliflozin 10 mg/d, and the control group was given the same dose of placebo. Other treatments were the same, and both groups of patients received individualized supportive treatment. The difference in auxiliary medication between the two groups was kept as small as possible, and the blood pressure, blood sugar, and lipid levels were kept relatively stable according to the specific conditions of the patients.

纳入标准：

Inclusion criteria

排除标准：

1.有明确的继发因素导致的IgA肾病； 2.入组前6个月内接受糖皮质激素或免疫抑制剂治疗者； 3. eGFR<15ml/min/1.73m^2 或肾脏替代治疗者； 4.入组前已经接受SGLT2i药物治疗者，或对达格列净或其他SGLT2i过敏者； 5.急性肾损伤、恶性肿瘤、严重呼吸道疾病、酒精成瘾者； 6.有低血糖或酮症酸中毒史者； 7.孕妇或哺乳期患者； 8.依从性差、不能配合的患者。

Exclusion criteria：

1.There is a clear secondary cause of IgA nephropathy; 2.Receiving glucocorticoid or immunosuppressant therapy within 6 months prior to enrollment; 3. eGFR<15ml/min/1.73m^2 or renal replacement therapy ; 4.SGLT2i treatment before the research, or allergic to SGLT2i; 5.AKI, malignant tumor, severe respiratory disease, alcohol addict; 6.History of hypoglycemia or ketoacidosis; 7.Pregnant or nursing patients; 8.Patients with poor compliance and inability to cooperate.
