研究目的:

通过比较DV-CTA 与支气管镜检查儿童气管软化结果的一致性情况,从而为临床提供一种无创、高效、一站式检查的新兴手段,除了定量评估气管软化程度之外,同时还可对心血管结构、纵隔和肺进行血管造影评估。对那些无法行支气管镜检查但又需要初步评估气管软化程度的患儿提供一种无创且高效的替代方案  

Objectives of Study:

To compare the consistency of DV-CTA and bronchoscopy in the detection of tracheomalacia in children, so as to provide a new non-invasive, efficient and one-stop examination method for clinical practice. In addition to quantitative assessment of tracheomalacia, it can also perform angiographic evaluation of cardiovascular structures, mediastinum and lung. To provide a noninvasive and efficient alternative for children who cannot undergo bronchoscopy but need an initial assessment of tracheomalacia

纳入标准:

Inclusion criteria

排除标准:

1)临床症状严重,如呼吸频率过快、气管插管、气管切开、氧饱和度过低 等;2)造影剂过敏及严重肝肾功能障碍;3)DV-CTA 扫描前行纤维支气管镜检查或 DV-CTA 扫描后 90 天内未行纤维支气管镜检查

Exclusion criteria:

1) Severe clinical symptoms, such as rapid respiratory rate, tracheal intubation, tracheotomy, low oxygen saturation, etc. 2) Contrast agent allergy and severe liver and kidney dysfunction; 3) fiberoptic bronchoscopy before DV-CTA scan or no fiberoptic bronchoscopy within 90 days after DV-CTA scan

气管塌陷面积百分比≥50%时诊断气管软化

Tracheal collapse diagnostic tracheomalacia area percentage of 50% or higher

指标试验(即本研究的待评估诊断试验,无论为方法、生物标志物或设备,均请列出名称):

对气管可见明显狭窄段的病例,测量狭窄段气管5个层面的气管横截面积;对气管未见明显狭窄段的病例,测量5个固定层面(胸廓入口、无名静脉水平、主动脉弓水平、左右肺动脉起始水平和气管隆突水平)的气管横截面积。对同一病例不同呼吸期相的图像测量应保证同一层面。根据公式:塌陷面积百分比=(最大横截面积-最小横截面积)/最大横截面积×100%,计算出每个测量层面在呼吸周期中气管塌陷面积百分比,选取5个层面中塌陷面积百分比的最大值作为该病例气管塌陷面积百分比,当气管塌陷面积百分比≥50%时诊断气管软化。最后,将同一病例完成DV-CTA 与纤维支气管镜检查的气管软化程度进行对照分析

Index test:

For the cases with obvious tracheal stenosis, the cross-sectional area of the tracheal stenosis at 5 layers was measured. For patients without obvious stenosis of the trachea, the cross-sectional area of the trachea was measured at 5 fixed levels (thoracic entrance, inonymous vein level, aortic arch level, starting level of left and right pulmonary arteries and tracheal carina level). The image measurement of different respiratory phases in the same patient should ensure the same plane. According to the formula: Subsidence area percentage = (maximum cross-sectional area - minimum cross-sectional area)/maximum cross-sectional area by 100%, calculated each measurement level in the respiratory cycle trachea collapse area percentage, select a maximum of five levels in the subsidence area percentage as the cases of tracheal collapse area percentage, when tracheal collapse diagnostic tracheomalacia area percentage of 50% or higher. Finally, the degree of tracheomalacia was compared between DV-CTA and fiberoptic bronchoscopy in the same case
