研究目的:

本研究拟基于前期构建好的3 万例队列的临床资料和基因信息建立老年糖尿病新型分型方法,并建立精准分型预测模型。其次,通过前瞻性干预性、非随机对照试验,在医疗机构随访老年糖尿病患者心肾结局,验证预测模型以及干预模式的有效性。最终建立在线新诊断老年糖尿病分型诊疗决策系统,该系统可以依据分层、分型结果主动干预指导用药并预测心肾并发症的风险。  

Objectives of Study:

In this study, we propose to establish a novel typing method for geriatric diabetes mellitus based on the clinical data and genetic information of a pre-constructed cohort of 30,000 cases, and to establish a prediction model for accurate typing. Secondly, the validity of the prediction model as well as the intervention model will be verified through a prospective interventional, non-randomised controlled trial to follow up the cardiac and renal outcomes of elderly diabetic patients in healthcare institutions. Finally, an online decision-making system for typing newly diagnosed older adults with diabetes will be established, which can proactively intervene to guide medication and predict the risk of cardiac and renal complications based on the stratification and typing results.

Inclusion criteria

排除标准:

(1)诊断糖尿病时间超过3年或同时应用超过1种降糖药物; (2)根据《中国老年糖尿病指南(2021版)》进行分层,结果为“group 3”(即健康等级为“差”)的患者; (3)合并严重心脑血管疾病; (4)危急重症患者; (5)合并慢性肾脏病; (6)患有精神疾病等不能签署知情同意的患者; (7)拒绝参加调查随访者。

Exclusion criteria:

(1) Diagnosis of diabetes mellitus for more than 3 years or concurrent use of more than 1 hypoglycaemic drug; (2) Patients stratified according to the "China Diabetes Guidelines for the Elderly (2021 Edition)", and the result is "group 3" (i.e., the health grade is "poor"); (3) Combined serious cardiovascular and cerebrovascular diseases; (4) Critically ill patients; (5) Patients with chronic kidney disease; (6) Patients with mental illnesses, etc., who cannot sign informed consent; (7) Those who refused to participate in the survey follow-up.

干预措施:

精准分型干预组在3家医院进行,入组患者进行分层和分型后进行精准的个体化治疗。具体分型如下:1. 以胰岛素缺乏为主要特征的糖尿病,2. 以胰岛素抵抗为主要特征的糖尿病,3. 以肥胖为主要特征的糖尿病。分型对应干预措施如下:1. 以胰岛素缺乏为主要特征的糖尿病,推荐改善生活方式的基础上进行胰岛素治疗。2.以胰岛素抵抗为主要特征的糖尿病,推荐改善生活方式的基础上进行二甲双胍联合SGLT-2i和改善生活方式治疗。改善生活方式方法参考2020年版中国2型糖尿病防治指南如下:① 使超重或肥胖个体体重指数达到或接近24kg/m2 ,或体重至少下降7%;②每日饮食总热量至少减少400-500kcal(1kcal=4.184kJ),超重或肥胖者应减少500-750kcal;③饱和脂肪酸摄入占总脂肪酸摄入的30%以下;每人每天食用盐的总量不超过5g;④ 中等强度体力活动至少保持在150min/周。3.以肥胖为主要特征的糖尿病,建议改善生活方式的基础上二甲双胍联合GLP-1进行干预。

Intervention:

The precise typing intervention group was conducted in three Hospitals, and the enrolled patients were stratified and typed for precise individualised treatment. The specific types were as follows: 1. diabetes mellitus mainly characterised by insulin deficiency, 2. diabetes mellitus mainly characterised by insulin resistance, 3. diabetes mellitus mainly characterised by obesity. The corresponding interventions are as follows: 1. for diabetes mellitus mainly characterised by insulin deficiency, insulin therapy based on lifestyle improvement is recommended; 2. for diabetes mellitus mainly characterised by insulin resistance, metformin combined with SGLT-2i and lifestyle improvement is recommended based on lifestyle improvement. Lifestyle improvement methods refer to the 2020 edition of the Chinese guidelines for the prevention and treatment of type 2 diabetes mellitus as follows: ① Make the body mass index of overweight or obese individuals reach or approach 24kg/m2 , or weight loss of at least 7%; ② Reduce the total calories in the diet by at least 400-500kcal per day (1kcal=4.184kJ), and those who are overweight or obese should reduce the total calories by 500-750kcal; ③ Saturated fatty acid intake accounted for the proportion of total fatty acid intake. Fatty acid intake accounts for less than 30% of the total fatty acid intake; the total amount of salt per person per day does not exceed 5g; ④ moderate-intensity physical activity should be maintained at least 150min/week. 3. diabetes mellitus with obesity as the main feature, it is recommended to improve the lifestyle based on metformin combined with GLP-1 intervention.
