研究目的：

1、构建较为完善的护士与照顾者共管照护干预方案，形成可以复制和推广的护理路径，提升患者生存质量，改善医疗机构的医护服务质量。 2、观察该护理模式的效果，了解照顾者在患者康复转归过程中发挥作用机制，探索提升护理服务质量的有效路径。  

Objectives of Study：

1. A relatively complete nurse-caregiver co-management care intervention plan will be constructed to form a nursing path that can be replicated and promoted, to improve the quality of life of patients and improve the quality of medical care services in medical institutions. 2. The effect of the nursing model will be observed to understand the mechanism of caregivers playing a role in the process of patient rehabilitation and to explore effective ways to improve the quality of nursing services.

纳入标准：

Inclusion criteria

排除标准：

1.病人排除标准：（1）失访或者中途退出；（2）有精神病史或严重意识障碍，影响正常沟通交流；（3）合并严重肝、肾、肺疾病以及心力衰竭、恶性肿瘤；（4）出院后在社区或康复机构接收治疗；（5）病情不稳定有加重倾向。；
2.照顾者排除标准：（1）不愿意参加和配合本研究；（2）未完成研究而退出；（3）照顾者在照顾期间发生重大疾病或应激事件。；

Exclusion criteria：

1.Patient exclusion criteria: (1) loss of follow-up or withdrawal in the middle of the process； (2) history of mental illness or severe impairment of consciousness, affecting normal communication； (3) combined with severe liver, kidney and lung diseases, heart failure, malignant tumors；(4) treatment in the community or rehabilitation institutions after discharge； (5) unstable condition with aggravation tendency.;
2.Caregiver exclusion criteria: (1) unwilling to participate in and cooperate with this study； (2) withdrawal without completion of the study； (3) major illness or stressful event during the caregiver's care.;
