评估迷走神经刺激治疗难治性抑郁症的有效性和安全性  

Objectives of Study：

To evaluate the safety and efficacy of Vagus Nerve Stimulation (VNS) in treatment-resistant depression (TRD)

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

1) 2名主治医师经过系统临床评估，符合DSM-IV其它精神疾病诊断； 2) 既往有自杀行为或现有强烈自杀倾向的患者； 3) 伴有精神病性症状的抑郁发作（妄想、幻觉）； 4) 既往有躁狂或轻躁狂发作； 5) 筛选前1年内按照DSM-IV诊断标准诊断酒精或药物滥用和依赖者； 6) 怀孕及哺乳期的妇女、在试验期间不能采取适宜的避孕措施的患者； 7) 既往存在重大躯体疾病者； 8) 以往左侧颈部有迷走神经切断史； 9) 存在有神经系统疾病者，有运动、感觉或认知功能受损或需要间断或长期服用药物的患者（例如：帕金森病、偏头痛、亨廷顿病，脑卒中、颅脑外伤）； 10) 胰岛素依赖性糖尿病患者； 11) 有呼吸睡眠暂停综合征患者； 12) 正在或预计在观察周期内将会接受强电磁治疗或检查的患者； 13) 确诊冠心病且既往有过心绞痛，心肌梗塞或者目前需要服用药物治疗者（服用降脂药物者除外）； 14) 有心脏起搏器或除颤器或其他植入性仪器者； 15) 目前使用抗凝血药物者； 16) 严重心肺功能不全者。

Exclusion criteria：

1) 2 physicians attending the system were evaluated clinically, which were in accordance with DSM-IV diagnosis of other mental disorders;
2) patients with previous suicidal behaviors or those with strong suicidal tendencies;
3) depressive episodes accompanied by psychotic symptoms (delusions, hallucinations);
4) with a history of mania or hypomania;
5) Diagnosis of alcohol or substance abuse and dependence in accordance with DSM-IV diagnostic criteria within 1 year prior to screening;
6) Pregnant and lactating women who can not take appropriate contraception during the trial;
7) Those with previous major physical diseases;
8) a history of vagotomy in the left neck;
9) Patients with neurological disorders who have impaired motor, sensory or cognitive function, or need intermittent or long-term use of drugs (eg Parkinson's disease, migraine, Huntington's disease, stroke, traumatic brain injury);
10) insulin-dependent diabetes mellitus;
11) patients with respiratory sleep apnea syndrome;
12) patients who are undergoing or are expected to undergo intensive electromagnetic therapy or examination during the observation period;
13) diagnosed coronary heart disease and previous angina, myocardial infarction or the need to take medication (except taking lipid-lowering drugs);
14) have pacemakers or defibrillators or other implanted instruments;
15) current use of anticoagulants;
16) Serious cardiopulmonary insufficiency.
