研究目的：

本研究将以患者为中心、护士为主导，基于CITM的自我管理模式应用于脑卒中患者的临床护理干预中，探讨该模式综合应用效果。  

Objectives of Study：

In this study, the patient-centered, nurse-led, CITM-based self-management model was applied to the clinical nursing interventions of stroke patients to explore the effect of the comprehensive application of the model.

纳入标准：

Inclusion criteria

排除标准：

（1）曾参加过其他自我管理培训项目，正在参加其他研究项目者。 （2）合并其他严重疾病（恶性肿瘤、严重肝肾功能不全、严重外伤、呼吸功能衰竭、严重心律失常、频繁心绞痛、心衰、心肌梗塞等患者），盲、聋、哑患者。 （3）意识障碍或有认知障碍者，简易智力测试量表（AMT）＜8分，或有精神障碍者、患有感觉性失语无法进行沟通者。

Exclusion criteria：

(1) Those who have participated in other self-management training programs and are participating in other research programs. (2) Those who have a combination of other serious diseases (patients with malignant tumors, severe hepatic or renal insufficiency, severe trauma, respiratory failure, severe cardiac arrhythmia, frequent angina pectoris, heart failure, myocardial infarction, etc.), and those who are blind, deaf, or mute. (3) Persons with impaired consciousness or cognitive impairment, with a score of <8 on the Abbreviated Mental Test Scale (AMT), or persons with psychiatric disorders, or persons suffering from sensory aphasia who are unable to communicate.
