探讨基于SCT-HAPA-FFT模型的脑卒中幸存者活动促进干预方案对脑卒中幸存者自我效能感、功能锻炼依从性、运动功能、生活质量的干预效果。  

Objectives of Study：

To investigate the intervention effects of an activity promotion intervention program for stroke survivors based on the SCT-HAPA-FFT model on self-efficacy, functional exercise adherence, motor function, and quality of life of stroke survivors.

纳入标准：

Inclusion criteria

排除标准：

（1）急性传染性疾病患者； （2）合并其他脑器质性疾病，伴有其他严重躯体疾病； （3）心肺肾肝功能严重不全等需绝对卧床休息者； （4）有精神疾病史或存在认知障碍； （5）有酒精或者其他精神活性物质滥用或者依赖史。

Exclusion criteria：

(1) Patients with acute infectious diseases; (2) Combined with other organic brain diseases, accompanied by other serious physical diseases; (3) People with severe cardiopulmonary, renal and hepatic insufficiency, etc. requiring absolute bed rest; (4) History of mental illness or cognitive impairment; (5) History of alcohol or other psychoactive substance abuse or dependence.

在对照组的基础上，由接受过统一培训的医务人员对患者实施基于SCT-HAPA-FFT模型的脑卒中幸存者活动促进的干预。根据实际情况采取线上（微信、腾讯会议）和线下（病房和健教室）相结合的形式干预8次，每次干预包括干预时间、干预主题、干预目标、干预内容、家庭作业内容。干预内容主要包括：健康教育（PPT、视频、手册）、康复活动训练、定期家访、经验分享、线上（微信打卡、言语鼓励）、线下（现场奖励）等。

On the basis of the control group, the intervention of activity promotion for stroke survivors based on the SCT-HAPA-FFT model was implemented to the patients by medical personnel who had received unified training. A combination of online (WeChat, Tencent conference) and offline (ward and health classroom) interventions were conducted 8 times according to the actual situation, and each intervention included intervention time, intervention theme, intervention goal, intervention content, and homework content. The intervention content mainly includes: health education (PPT, video, manual), rehabilitation activity training, regular home visits, experience sharing, online (WeChat punch card, verbal encouragement), and offline (on-site rewards).
