主要目的 (1)研究基于多模态闭环反馈运动想象脑机接口是否对亚急性期脑卒中手功能康复有促进作用。 (2)探究基于多模态闭环反馈运动想象脑机接口是否通过调节皮层激活、改善质脊髓束的完整性、影响脑区的激活和重塑脑区功能连接改善亚急性期脑卒中手功能,同时进一步揭示多模态闭环反馈运动想象脑机接口与亚急性期脑卒中手功能之间的相关性。 次要目的 通过对亚急性期脑卒中手功能障碍患者进行BCI系统干预后的分类准确率百分比(%CA)评价BCI的性能和用户体验探究该系统的准确性、可靠性、可用性和可接受性。 探索性目的 探究多模态闭环反馈运动想象脑机接口在亚急性期脑卒中手功能康复中的作用及神经机制  

Objectives of Study:

Main purpose (1) To investigate whether multi-modal closed-loop feedback motor imagination brain-computer interface can promote hand function rehabilitation in sub-acute stroke. (2) To explore whether the multi-modal closed-loop feedback motor imagination brain-computer interface can improve the hand function of sub-acute stroke by regulating cortical activation, improving the integrity of the plastic-spinal tract, affecting the activation of brain regions and reshaping the functional connections of brain regions, and further reveal the correlation between the multi-modal closed-loop feedback motor imagination brain-computer interface and hand function of sub-acute stroke. Secondary purpose The classification accuracy percentage (%CA) after BCI system intervention for sub-acute stroke patients with hand dysfunction was used to evaluate the performance of BCI and user experience to explore the accuracy, reliability, availability and acceptability of the system. Exploratory purpose To explore the role and neural mechanism of multimodal closed-loop feedback motor imagination brain-computer interface in hand function rehabilitation of subacute stroke

纳入标准:

Inclusion criteria

排除标准:

①大脑皮层梗死或数次梗死、脑出血;②患侧肌张力较高(改良Ashworth 分级>2级)或疼痛(视觉模拟量表>4分);③有癫痫病史、一级亲属中有特发性癫痫病史及使用致痫药物患者;④戴有起搏器、颅内有金属植入物,或有颅骨缺陷患者;⑤严重注意、视觉及交流障碍或其他干扰研究目的的体征;

Exclusion criteria:

① cerebral cortex infarction or several infarction, cerebral hemorrhage; ② The affected side has high muscle tone (modified Ashworth scale >2) or pain (visual analog scale >4); ③ Patients with a history of epilepsy, first-degree relatives with a history of idiopathic epilepsy and the use of epileptogenic drugs; Patients who wear pacemakers, have metal implants in the skull, or have skull defects; Severe attention, visual and communication impairments, or other signs that interfere with the purpose of the study;

基于运动想象脑电的智能脑机接口系统主要由运动想象脑电采集系统、手功能气泵和配套训练软件3部分组成。训练前向患者充分讲解训练流程,嘱患者在训练过程中避免主动眨眼、咳嗽、咀嚼、头部和身体移动;协助患者取舒适坐位,距离提示屏幕50 cm左右;佩戴脑电帽和手功能气泵;患者在视、听文字提示信息后完成MI任务,由脑电系统进行判定,如超过设定的脑电强度变化阈值则计算机给予一次奖励性视觉与听觉反馈,手功能气泵给予一次触觉、运动觉反馈,失败则不予反馈。

The intelligent brain-computer interface system based on motion image EEG is mainly composed of three parts: motion image EEG acquisition system, hand function air pump and supporting training software. Fully explain the training process to the patient before training, and ask the patient to avoid actively blinking, coughing, chewing, head and body movement during training; Assist the patient to take a comfortable seat, about 50 cm away from the prompt screen; Wear EEG cap and hand function air pump; Patients complete MI tasks after visual and listening to text prompts, which are judged by the EEG system. If the EEG intensity changes exceed the set threshold value, the computer will give a rewarding visual and auditory feedback, and the hand functional air pump will give a tactile and kinesthetic feedback, and no feedback will be given if they fail.
