纳入标准：

Inclusion criteria

排除标准：

（1）患者排除标准： ①处于急性发病期或病情较重，不适合参与；②有精神疾患；③意识障碍；④言语和听力障碍。 （2）照顾者排除标准：①言语和听力障碍；②与患者为雇佣关系。

Exclusion criteria：

(1) For patients: ① In an acute stage of illness or in a serious condition that makes participation unsuitable; ② with mental illness; ③ with impaired consciousness; ④ speech and hearing impairment. (2) For carers: ① speech and hearing impairment; ② employment relationship with the patient.

采取冠心病二元疾病管理干预策略进行赋能干预。干预的实施者为研究团队中的护理成员，均为心血管病专科护士。研究团队在患者/照顾者的手机上安装前期研发的mHealth应用，首先通过测量二元照顾责任、二元疾病管理任务，明确冠心病患者和照顾者各自的支持重点；进而，在软件应用内，研究对象进入项目后进行各个方面的疾病管理行为测评，研究人员根据测评结果为赋能对象提供能力提升目标，软件自动在能力不足的方面进行指导信息推送，直至患者/照顾者具备良好的疾病管理能力。

Intervention：

An empowering intervention was conducted using a dyadic patient-carer disease management intervention strategy for coronary heart disease. The intervention was implemented by nursing members of the study team, all of whom were cardiovascular disease specialty nurses. The research team installed the pre-developed mHealth application on the patient/carer's mobile phone, firstly, through the measurement of binary caregiving responsibilities, dyadic disease management tasks, to clarify the respective support priorities of patients with coronary artery disease and carers; then, within the software application, the research subjects entered the program to conduct various aspects of the disease management behavioral assessment, and the researcher provided competency enhancement for the empowered subjects based on the results of the assessment Based on the assessment results, researchers provide empowerment targets with capacity enhancement goals, and the software automatically pushes guidance information in areas where capacity is lacking until the patient/caregiver is well equipped with disease management skills.

数据采集和管理（说明：数据采集和管理由两部分组成，一为病例记录表(Case Record Form, CRF)，二为电子采集和管理系统(Electronic Data Capture, EDC)，如ResMan即为一种基于互联网的EDC：

1.使用病例记录表(CRF)进行初始数据采集，每位受试者一份CRF，由研究团队成员采集数据并记录，CRF专柜上锁保存于复旦大学附属中山医院心内科办公室。 2.使用电子采集和管理系统(EDC)，由研究人员在EDC系统中建立数据库，将数据编码，由研究人员录入EDC系统中进行保存，研究人员可从数据库中导出分析。EDC系统设置密码，只允许研究组内成员进入使用。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture：

1. Initial data collection was performed using a case record form (CRF), one CRF for each subject, with data collected and recorded by members of the research team, and the CRF was kept in a locked cabinet in the office of the Department of Cardiology, Zhongshan Hospital, Fudan University. 2. Electronic collection and management system (EDC) was used, the database was set up by the researchers in the EDC system, the data were coded and entered by the researchers into the EDC system for storage, and the researchers could export and analyze the data from the database.The EDC system was password-set, and only the members of the research team were allowed to enter and use the system.
