① 解析毁损肺、肺空洞组织、结核性肉芽肿中心区域、边界区域和邻近正常组织免疫微环境的异质性,绘制各部分的单细胞图谱,发现与Mtb播散密切相关的细胞亚群、关键调控网络和因子; ② 分析结核性肉芽肿中心区域、边界区域、邻近正常组织的TCR克隆以及V(D)J基因的表达变化; ③ 探讨肺结核和Mtb侵袭播散过程中细胞异质性的形成,初步揭示Mtb免疫逃逸、肺空洞和毁损肺形成的机制,寻找免疫标志物和治疗新靶点。 |

Objectives of Study: |
1. To investigate the heterogeneity of the immune microenvironment in damaged lung tissues, including pulmonary cavity tissue, central area, border area, and adjacent normal tissue of tuberculous granuloma. Additionally, to construct a single-cell map for each region in order to identify cell subsets, key regulatory networks, and factors closely associated with Mtb dissemination. 2. Analyzing TCR clones and V (D) J gene expression in the central region, border region, and adjacent normal tissues of tuberculous granuloma. 3. Exploring the development of cellular heterogeneity during tuberculosis invasion and dissemination by elucidating the mechanisms underlying Mtb immune evasion as well as pulmonary cavity formation and lung destruction. Furthermore, identifying immune markers and novel treatment targets. |

排除标准: |
①不配合本研究; ②人类免疫缺陷病毒阳性; ③合并其他免疫功能低下的疾病; ④长期使用全身激素或使用免疫抑制剂患者; ⑤如果术前痰菌阴性,胸部X线摄影、CT检查诊断为肺结核,但是病理检查诊断不支持结核病者。 ⑥可能影响肺损伤和免疫微环境的相关性肺部和系统性疾病,以下是一些常见的相关性肺部和系统性疾病的分类和例子: a) 自身免疫性疾病:由免疫系统错误地攻击身体自身组织引起的,可能导致肺部损伤和免疫微环境改变。例子包括系统性红斑狼疮、类风湿性关节炎、硬皮病等。 b) 结缔组织病:这是一组累及结缔组织的疾病,可能对肺部产生直接或间接的影响。其中最常见的是类风湿性关节炎、系统性硬化病和混合性结缔组织病。 c) 炎症性肉芽肿性疾病:这些疾病以炎症性肉芽肿形成为特征,可能影响肺部和免疫微环境。例子包括肉芽肿性多动脉炎、结节性多动脉炎等。 d) 免疫相关的肺部感染:某些免疫系统相关疾病可能增加患者患上肺部感染的风险,导致肺部损伤。例如,免疫抑制剂使用者可能更容易患上细菌、真菌或病毒感染。 e) 其他系统性疾病相关的肺部损伤:一些系统性疾病可能导致肺部损伤或影响免疫微环境。例如,糖尿病、高血压和心血管疾病可能增加肺部损伤的风险。 |

Exclusion criteria: |
1. Do not participate in this study. 2. Human immunodeficiency virus positive status. 3. Complicated with other conditions that compromise the immune system. 4. Long-term use of systemic steroids or immunosuppressive medications. 5. If the preoperative sputum test was negative, but chest X-ray and CT scan indicated pulmonary tuberculosis, the pathological diagnosis did not confirm tuberculosis. 6. Related pulmonary and systemic diseases that may impact lung injury and the immune microenvironment include: a) Autoimmune diseases: autoimmune response against one's own tissues resulting in lung damage and alterations in the immune microenvironment (e.g., systemic lupus erythematosus, rheumatoid arthritis, scleroderma). b) Connective tissue diseases: disorders affecting connective tissue directly or indirectly impacting lungs (e.g., rheumatoid arthritis, systemic sclerosis, mixed connective tissue disease). c) Inflammatory granulomatous diseases: inflammation leading to granuloma formation affecting both lungs and the immune microenvironment (e.g., polyarteritis granularis, polyarteritis nodosa). d) Immune-related lung infections: certain conditions increasing risk of lung infections causing damage to lungs (e.g., individuals using immunosuppressants more susceptible to bacterial, fungal or viral infections). e) Other systemic disease-associated lung damage: some diseases causing lung damage or impacting immune microenvironment such as diabetes mellitus, hypertension and cardiovascular disease increasing risk of lung injury. |
