研究目的：

实施慢性心衰患者自护和照护者自护贡献双元干预方案，验证其对患者自护、照护者自护贡献、患者生活质量、照护者生活质量和患者再入院和死亡的影响。  

Objectives of Study：

This study will implement a dyadic intervention program on patients with chronic heart failure and their family caregivers, to validate its effect on patient self-care, caregiver self-care contribution, patient quality of life, caregiver quality of life, and patient readmission and death.

纳入标准：

Inclusion criteria

排除标准：

患者排除标准：①近一个月内有急性心肌梗死发作；②有其他系统严重合并症或并发症，如恶性肿瘤、肝肾肺等脏器的功能衰竭、精神障碍等。 照护者排除标准：①领取照护报酬；②自身有严重躯体疾病（如恶性肿瘤、重要脏器衰竭等），认知障碍或精神疾病等。

Exclusion criteria：

Patient exclusion criteria: (1) acute myocardial infarction within the past month; (2) other systematic serious comorbidities or complications, such as malignant tumors, liver, kidney, lung and other organs failure, mental disorders and so on. Exclusion criteria for caregivers: (1) receiving care remuneration; (2) having serious physical diseases (such as malignant tumors, vital organ failure, etc.), cognitive disorders, or mental illnesses.

随机方法（请说明由何人用什么方法产生随机序列）：

采用区组随机法对符合标准的心衰患者和照护者进行随机分组，随机序列将由一名研究助理（没有参与本研究的参与者招募、干预实施或结果评估）使用在线程序生成。在每个区组中，两个字母“A”和“B”分别表示干预组和对照组。按顺序编号后，表明分组的标签卡片会放进不透明的密封信封中，然后由对不知标签的研究助理进行信封发放。根据参与者进入研究的顺序，向他们提供信封。

Randomization Procedure (please state who generates the random number sequence and by what method)：

Patients with heart failure and caregivers who meet the criteria will be randomized using block randomization. Randomisation sequences are generated using an online programme by a research assistant who is not involved in the participant recruitment, intervention implementation, or outcome assessments of the present study. In each block, the two letters 'A' and 'B' indicate the intervention and control group, respectively. Sequentially numbered, labelled cards indicating group allocation will be inserted into opaque sealed envelopes by the research assistant blinded to the number labelling. An envelope is provided to the participants according to their sequence of entry into the study.
