研究目的：

（1）证明自由饮水训练可以超早期实施。 （2）证明自由饮水训练对于吞咽障碍康复的安全性和有效性。 （3）探讨自由饮水训练过程的吞咽改善机制。  

Objectives of Study：

(1) Prove that free drinking water training can be implemented very early. (2) To demonstrate the safety and efficacy of free drinking water training for the rehabilitation of swallowing disorders. (3) To explore the swallowing improvement mechanism in the process of free drinking training.

药物成份或治疗方案详述：

纳入标准：

Inclusion criteria

排除标准：

①咽肌失迟缓症（VFSS筛查）；②口腔、鼻咽部等原发病变导致的吞咽障碍；③合并严重心、肝、肾脏等重要脏器功能衰竭；④妊娠；⑤有精神行为异常；⑥不能或不愿意配合；⑦未签署知情同意书。

Exclusion criteria：

(1) Pharyngeal muscle loss syndrome (VFSS screening); (2) Swallowing disorders caused by primary lesions such as oral cavity and nasopharynx; (3) Combined with severe heart, liver, kidney and other important organ failure; (4) pregnancy; (5) Have abnormal mental behavior; (6) Unable or unwilling to cooperate; (7) Failure to sign informed consent.

①根据容积黏度吞咽测试（VVST）吞咽安全有效性结果，患者及家属知情同意后，即行摄食训练；②常规口腔护理，2次/日；③患者取直立坐位（900）或坐轮椅；④训练用食物的制作-使用舒食素G（日本NUTRI公司生产，1.5g／袋）1袋加入200ml开水，搅拌冷却后成水凝胶状备用，无需加入任何食物；⑤将食物送至健侧舌后根或颊部；⑥摄食训练的一口量，从3～5ml开始，每次20ml，频率：每日2次，逐渐递增至每次100～200ml。⑦如果患者在5min内可以进食训练用食物200ml，连续3d没有呛咳和隐性误吸，经患者、家属及管床医生同意，拔除鼻胃管。

干预措施代码：

Intervention：

(1) According to the results of the volumetric viscosity swallowing test (VVST) swallowing safety and effectiveness, the patient and his family will be given informed consent to carry out feeding training; (2) Routine oral care, 2 times/ (3) The patient should sit upright (900) or use a wheelchair;(4) Preparation of training food - use Shu Shisu G (produced by NUTRI in Japan, 1.5g/bag) 1 bag with 200ml of boiling water, stir and cool it into a hydrogel form for later use, no need to add any food;(5) Send food to the root of the tongue or cheeks on the healthy side;(6) One bite of feeding training, starting from 3~5ml, 20ml each time, frequency: 2 times a day, gradually increase to 100~200ml each time. (7) If the patient can eat 200ml of training food within 5 minutes, and there is no choking and silent aspiration for 3 consecutive days, the nasogastric tube should be removed with the consent of the patient, family members and the doctor in charge of the bed.

自由饮水训练①用物准备：洁净的容器（缺口杯）、纯净水、电动牙刷、血氧饱和度监测仪；②患者取坐轮椅（调整轮椅角度600），给予高质量的口腔护理（电动牙刷+巴氏刷牙法）；③咳嗽训练2-3次，有效清理痰液；④调整颈部姿势：前屈位；⑤测定患者饮水的一口量（用10ml量杯测定患者的一口饮水量，测定时将一口量控制在2-5ml，不引起呛咳为准。）；⑥缺口杯内装90mL纯净水，饮水量以安全一口量为基础；⑦口令指导：一口量的水在口腔停留1-2秒，听从指令后吞咽。⑧吞咽后，观察患者有无呛咳、音质和血氧饱和度的变化；⑨不限制饮水次数，直至90ml水喝完。频率：每日2次。⑩逐渐递增至每次100～200ml。如果患者在5min内可以自由饮水200ml，连续3d没有呛咳和隐性误吸，经患者、家属及管床医生同意，拔除鼻胃管。

Intervention：

Free drinking water training (1) Preparation of materials: clean container (notched cup), purified water, electric toothbrush, blood oxygen saturation monitor; (2) The patient takes a wheelchair (adjusts the wheelchair angle 600) and gives high-quality oral care (electric toothbrush + pasteur brushing) ;(3) Cough training 2-3 times to effectively clean up sputum; (4) Adjust the neck posture: forward bending; (5) Measure the amount of water the patient drinks (use a 10ml measuring cup to measure the patient's mouthful of water, and control the amount of one mouth at 2-5ml during the measurement, which does not cause choking. ;(6) The notched cup is filled with 90mL of purified water, and the amount of water to be drunk is based on a safe mouthful;(7) Password guidance: A mouthful of water stays in the mouth for 1-2 seconds, and swallows after following the instructions. (8) After swallowing, observe whether the patient has choking, voice quality and changes in blood oxygen saturation; (9) Do not limit the number of drinking times until 90ml of water is drunk. Frequency: 2 times daily. (10) Gradually increase to 100~200ml each time. If the patient can drink 200ml of water freely within 5 minutes, and there is no choking and silent aspiration for 3 consecutive days, the nasogastric tube will be removed with the consent of the patient, family members and the doctor who is in charge of the bed.
