研究目的：

（1）利用医联体专全团队，探索在社区卫生服务机构开展慢性阻塞性肺疾病肺康复的工作模式； （2）评价全科医生借助手机APP实施慢性阻塞性肺疾病的社区肺康复的效果。  

Objectives of Study：

(1) to explore the working mode of the pulmonary rehabilitation of chronic obstructive pulmonary disease in community health service institutions using the medical association specialized team; (2) to evaluate the effect of community lung rehabilitation on chronic obstructive pulmonary disease (COPD) by general practitioner using mobile phone APP.

药物成份或治疗方案详述：

 

Description for medicine or protocol of treatment in detail：

 

纳入标准：

Inclusion criteria

排除标准：

①有运动诱发的哮喘；②非慢阻肺引起的各种严重器官病变、严重原发性疾病、肺结核患者；③重度肺动脉高压；④严重心功能不全、肝肾功能不全患者；⑤学习能力理解低下；⑥正参加康复程序或其他相关康复研究项目者；⑦既往有运动后晕厥史；⑧伴有重度精神障碍的患者；⑨其他不适合参加肺康复的情况。

Exclusion criteria：

1. there were exercise-induced asthma;
2. all kinds of serious organ diseases, severe primary diseases and pulmonary tuberculosis patients caused by non - chronic obstructive pulmonary disease;
3. severe pulmonary hypertension;
4. severe cardiac insufficiency, liver and kidney insufficiency patients;
5. poor understanding of learning ability;
6. those who are participating in the rehabilitation procedures or other relevant rehabilitation research projects;
7. the history of syncope after exercise;
8. patients with severe mental disorder;
9. other circumstances that are not suitable for participation in the lung rehabilitation.
