研究目的：

国产盐酸右美托咪定注射液应用于老年脊椎手术的有效性和安全性的多中心、随机、安慰剂平行对照临床研究，为国产盐酸右美托咪定注射液术中应用于老年脊椎手术患者提供可靠依据和合适药物剂量。

Objectives of Study：

To evaluate the efficacy and safety of dexmedetomidine in older patients undergoing spinal surgery.

药物成份或治疗方案详述：

Description for medicine or protocol of treatment in detail：

纳入标准：

Inclusion criteria

排除标准：

(1) 已知盐酸右美托咪定或其他本研究用药过敏者；
(2) 长期应用麻醉性镇痛药、镇静剂或非甾体类抗炎药物者；
(3) 既往神经肌肉疾病史者；
(4) 既往内分泌系统疾病史者；
(5) 既往变态反应疾病史者；
(6) 既往颈椎手术史者；
(7) 既往有精神疾病史者；
(8) 术前HR<50次/分、SBP<100 mmHg或SBP>160mmHg或DBP>110mmHg、心脏传导或节律异常者；
(9) 预计手术时间<1h或>3h，或手术颈椎节段>3个，或出血>400ml；
(10) 观察期内需二次手术者；
(11) 肝肾功能障碍者；
(12) 术前存在困难气道者（包括解剖畸形、改良Mallampati评分>3等）；
（13）近三月内参加其他药物临床研究者；
（14）研究者认为不适宜参加临床试验者。

Exclusion criteria：

Patients were excluded if they met any of the following criteria: preoperative history of schizophrenia, epilepsy, Parkinsonism, or myasthenia gravis; inability to communicate in the preoperative period (coma, profound dementia, or language barrier); brain injury or neurosurgery; known preoperative left ventricular ejection fraction less than 30%, sick sinus syndrome, severe sinus bradycardia (<50 beats per min [bpm]), or second degree or greater atrioventricular block without pacemaker; serious hepatic dysfunction (Child-Pugh class C); serious renal dysfunction (undergoing dialysis before surgery); or low likelihood of survival for more than 24 h; difficult airway; operation time <1h or >3hs; blood loss>400ml;segment more than 3; secondary surgery; participate in other clinical trials in 3 months.
