研究目的：

本研究旨在进行一项随机对照研究，以探索高血压患者的数字化移动健康管理组，并评估其在社会经济方面的效益。同时，本研究计划构建一个互联的高血压移动管理平台系统，以建立一种创新的高血压防治范式和有效机制。该研究的目的是为制定高血压管理的新策略提供理论和研究依据，以改善当前高血压防治的现状和效果，并努力减轻疾病对社会的负担。通过此研究，我们希望能够为推进高血压防治提供创新的方法和改善患者管理的实践指导。  

Objectives of Study：

The aim of this study is to conduct a randomized controlled study to explore a digital mHealth management group for hypertensive patients and to assess its socioeconomic benefits. Meanwhile, this study plans to build an interconnected hypertension mobile management platform system in order to establish an innovative paradigm and effective mechanism for hypertension management. The aim of this study is to provide a theoretical and research basis for the development of new strategies for hypertension management in order to improve the current status and effectiveness of hypertension prevention and treatment, as well as to endeavor to reduce the burden of the disease on society. Through this study, we hope to provide innovative approaches to advancing hypertension control and practical guidance for improving patient management.

纳入标准：

Inclusion criteria

排除标准：

(1) 妊娠或哺乳期妇女； (2) 慢性收缩性心力衰竭失代偿期 (NYHA 心功能分级≥3 级) 、心源性休克等严重心血管并发症患者； (3) 严重胃肠道疾病、肝肾功能衰竭患者； (4) 癌症患者或五年之内接受过放射治疗或化学治疗患者； (5) 患有艾滋病和肺结核等传染性疾病； (6) 住院或患有其他需要立即住院治疗的严重疾病者； (7) 不良嗜好者：使用毒品或者酗酒者 (男性摄入酒精>80 克/天，女性>40 克/天) ； (8) 意识障碍、无法正常交流或任何精神疾病；生活不能自理或行动不便者; (9) 预期寿命<12个月者;

Exclusion criteria：

(1) Pregnant or lactating women; (2) Patients with severe cardiovascular complications such as chronic systolic heart failure in the decompensated stage (NYHA cardiac function class ≥3) and cardiogenic shock; (3) Patients with severe gastrointestinal diseases, liver and kidney failure; (4) Cancer patients or patients who have received radiation therapy or chemotherapy within five years; (5) Patients with infectious diseases such as AIDS and tuberculosis; (6) Those who are hospitalized or suffering from other serious diseases requiring immediate hospitalization; (7) Bad habits: drug use or alcoholism (alcohol intake >80 grams/day for men and >40 grams/day for women); (8) Persons with impaired consciousness, inability to communicate normally, or any mental illness; persons who are unable to take care of themselves or have limited mobility; (9) Persons with a life expectancy of <12 months;

数字化移动健康管理组：对入组患者按照目前国内的高血压临床诊疗指南进行治疗和管理的基础上针对监控指标予更严格的干预管理，除门诊随诊时的常规生活方式宣教(清淡饮食、戒烟、适量运动等)和常规药物干预之外，将建立数字化移动健康管理高血压，帮助实现干预对象血压、血糖和血脂严格达标。通过药物/血压测量信鸽提醒，药物/血压监测打卡模式，可视化数字监测，多元化健康宣教，自我日程安排，医患之间信息沟通，数字化高血压健康科普服务，血压报告分析，高血压健康资讯，个人信息中心，问卷列表综合移动健康管理防治高血压。数字化移动健康管理组的干预方案及干预目标值参见数字化移动健康管理组推荐方案。

干预措施代码：

Intervention：

On the basis of the treatment and management of the enrolled patients according to the current domestic clinical diagnosis and treatment guidelines for hypertension and more stringent intervention management for the monitoring indicators, in addition to the routine lifestyle education (light diet, smoking cessation, moderate exercise, etc.) and routine drug interventions in the outpatient clinic follow up, the digital mobile health management of hypertension will be set up, to help achieve the intervention of the object of the strict standard of blood pressure, blood glucose and blood lipids. Through medication/blood pressure measurement pigeon reminder, medication/blood pressure monitoring punch card mode, visual digital monitoring, diversified health education, self-scheduling, information communication between doctors and patients, digital hypertension health science service, blood pressure report analysis, hypertension health information, personal information center, questionnaire list integrated mobile health management to prevent and control hypertension. The intervention plan and target values of the digital mHealth management group can be found in the recommended plan of the digital mHealth management group.

常规治疗组：对入组患者按照目前国内的高血压临床诊疗指南进行治疗和管理，包括门诊随诊时的常规生活方式宣教 (清淡饮食、戒烟、适量运动等) 和常规药物干预推荐，交代患者按时服药和定期测量血压。不进行主动式的定期远程干预，仅于定期进行电话和微信随访记录受试者病情有无变化、记录用药方案及有无药物不良反应、是否出现终点 (结局) 事件。常规治疗组的干预目标值参见常规治疗组建议控制目标。

Conventional treatment group: In accordance with current domestic clinical guidelines for hypertension, enrolled patients received comprehensive management, including regular lifestyle education (such as dietary adjustments, smoking cessation, and moderate exercise) and routine pharmacological interventions during outpatient follow-up visits. Patients were advised to adhere to their medication regimen and undergo regular blood pressure monitoring. No active remote intervention was implemented; only phone and WeChat follow-ups were conducted to document changes in patients' conditions, record medication adherence and adverse drug reactions, as well as monitor endpoint events. The target control values for the conventional treatment group align with recommended control targets.
