研究目的：

1.探讨基于中医药+互联网的、医院主导的三级联动家庭心脏康复模式的安全性和有效性。 2.建立适合国情的“三级联动”家庭心脏康复管理模式和标准化流程，提高心血管等慢病的管理效率。 3.秉承东直门医院中医底蕴，在区域内不断提升和普及中西医结合心脏康复和“双心”医学的诊疗特色。  

Objectives of Study：

1.To investigate the safety and efficacy of a hospital-led, triple-linkage home-based cardiac rehabilitation model that integrates Traditional Chinese Medicine (TCM) with internet-based technologies. 2. To establish a "Triple-Linkage" home-based cardiac rehabilitation management model and standardized protocol suitable for the national context, thereby improving the management efficiency of cardiovascular and other chronic diseases. 3.To uphold the profound heritage of Traditional Chinese Medicine at Dongzhimen Hospital by continuously enhancing and popularizing the integrated Chinese-Western medicine approach to cardiac rehabilitation and the distinctive diagnosis and treatment features of "Double Heart" medicine within the region.

Inclusion criteria

排除标准：

1. 恶性心律失常患者 2. 不稳定心绞痛或心肌梗死急性期 3. 控制不佳的高血压 4. 心衰急性期 5. 不适宜参加心肺运动试验及运动康复的其他疾病

Exclusion criteria：

1. Patients with malignant arrhythmias 2. Unstable angina or acute myocardial infarction 3. Poorly controlled hypertension 4. Acute heart failure 5. Other conditions unsuitable for cardiopulmonary exercise testing or exercise rehabilitation

1. 药物处方（院内进行）：由心脏康复医师进行冠心病二级预防药物处方制定与调整； 2. 运动锻炼（院内进行）：医师根据心肺运动试验制定有氧运动处方，采用HRR法：目标心率=（最大心率-静息心率）×40%~60%＋静息心率；频率为3次/周，每次40–60分钟，共治疗24次； 3. 中医腹针治疗（院内进行＋社区培训）：根据心病病因病机的特点，总结出腹针治疗冠心病的基础方：君臣—引气归元（D）；佐—商曲（左）、气旁（左）、气穴（左）（M）；使—水分（S）；管针进针后不施手法，留针20分钟，每周两次； 4. 经穴体外反搏（院内进行）：取穴三阴交、阴陵泉、足三里、丰隆，治疗压力0.020~0.035 MPa，频率为每周1次，时间为30分钟/次。

1. Medication prescription (performed in-hospital): Development and adjustment of secondary prevention medications for coronary artery disease by a cardiac rehabilitation physician; 2. Exercise training (performed in-hospital): Aerobic exercise prescription based on cardiopulmonary exercise testing using the HRR method: target heart rate = (max heart rate - resting heart rate) × 40%~60% + resting heart rate; frequency: 3 times/week, 40–60 minutes per session, total 24 sessions; 3. Abdominal acupuncture (performed in-hospital + community training): Based on the pathogenesis of heart disease, a foundational acupuncture formula for coronary artery disease is used: Jun-Chen—Yinqi Guiyuan (D); Zuo—Shangqu (left), Qipang (left), Qixue (left) (M); Shi—Shuifen (S); needle insertion without manipulation, retention for 20 minutes, twice weekly; 4. Acupoint-based external counterpulsation (performed in-hospital): Acupoints selected—Sanyinjiao, Yinlingquan, Zusanli, Fenglong; treatment pressure: 0.020~0.035 MPa; frequency: once weekly, 30 minutes per session.
