研究目的：

1.构建母婴回应性照护培训项目。 2.将回应性照护方案应用于临床，探讨回应性照护对母婴依恋关系、母亲育儿胜任感、早产儿生长发育、再入院率及早产儿母亲敏感性的潜在影响，进一步评价方案有效性，并为方案的广泛推广奠定基础。  

Objectives of Study：

1.Develop a mother-infant responsive care training program. 2.Implement the responsive care intervention in clinical settings, investigate its potential impact on mother-infant attachment, maternal parenting competence, preterm infant growth and development, readmission rates, and maternal sensitivity to preterm infants. Further assess the effectiveness of the intervention and establish a foundation for widespread implementation.

Inclusion criteria

排除标准：

早产儿排除标准：①新生儿有先天畸形者；②严重早产儿合并症，如；重度支气管肺发育不良、重度窒息、核黄疽、重度缺血缺氧性脑病、Ⅳ级颅内出血、重度贫血等； 母亲排除标准：早产儿母亲患有重大疾病（如恶性高血压、恶性肿瘤等）或视听障碍；

Exclusion criteria：

Exclusion criteria for preterm infants: ① Newborns with congenital anomalies; ② Severe complications in preterm infants, such as severe bronchopulmonary dysplasia, severe asphyxia, kernicterus, severe hypoxic-ischemic encephalopathy, grade IV intracranial hemorrhage, severe anemia, etc. Exclusion criteria for mothers: Mothers of preterm infants with significant illnesses (such as malignant hypertension, malignant tumors, etc.) or sensory impairments.

接受常规的NICU早产儿的护理和随访，入院时向早产儿家属介绍NICU病房环境、探视制度，告知母乳喂养的好处、母乳的储存及运送，住院期间母乳接收时间。入院后给予保暖、抗感染、呼吸道管理等治疗护理，每周由主管医生向父母解释病情，如有病情变化随时与父母进行沟通。出院时由责任护士向父母进行出院宣教，包括居家环境、预防感染、沐浴、喂养、出院带药等，出院后按时随访。

The preterm infant receives standard care and follow-up in the Neonatal Intensive Care Unit (NICU). Upon admission, NICU staff introduce the unit's environment and visitation policies to the family, providing information on the benefits of breastfeeding, storage and transport of breast milk, and the schedule for breastfeeding during hospitalization. Treatment and care include warmth, infection prevention, and respiratory management. Weekly, the attending physician explains the infant's condition to parents, maintaining open communication about any changes. Upon discharge, the assigned nurse conducts discharge education for parents, covering home environment, infection prevention, bathing, feeding, medication administration post-discharge, and schedules follow-up visits.

Intervention：

Participate in the mother-infant responsive care training program on the basis of the control group During the NICU hospitalization of preterm infants, mothers undergo five sessions of responsive care training, combining online and offline components, as outlined in the training program. The training encompasses identifying behavioral cues in preterm infants under stress, understanding signs of infant stress, recognizing cues of interactive desires and overstimulation in preterm infants. Mothers are instructed on interactive caregiving, responsive feeding, and recognizing sleep and bowel habits of preterm infants. The mother-infant dyad will apply responsive care knowledge when visiting the NICU. In the week before preterm infant discharge, mothers and infants will participate in a family-centered rooming-in experience, allowing mothers to further apply responsive care knowledge in daily caregiving, consolidating their ability to provide responsive care to preterm infants.
