研究目的:

1.评估PLA2R结构域特异性抗体及表位扩散在PMN判断风险分层和预测蛋白尿缓解的临床价值,从抗原表位水平了解PMN免疫学发病机制的特点,为PMN个体化治疗寻找更精准的生物标志物; 2.明确PLA2R和HLA基因多态性与PMN发病的相关性,探索相关SNP的功能意义,从基因层面认识PMN发病机制的特点,为PMN精准治疗提供依据。  

Objectives of Study:

1.Evaluate the clinical significance of PLA2R domain-specific antibodies and epitope spreading in predicting risk stratification and predicting proteinuria remission in primary membranous nephropathy (PMN). Understand the immunopathological characteristics of PMN at the antigenic epitope level, aiming to identify more precise biomarkers for individualized treatment of PMN. 2.Clarify the correlation between PLA2R and HLA gene polymorphisms and the onset of PMN. Explore the functional significance of relevant SNPs at the genetic level to understand the characteristics of the pathogenesis of PMN. Provide a basis for precision treatment of PMN.

Description for medicine or protocol of treatment in detail:

Treatment plan based on KDIGO guidelines for membranous nephropathy and the patient's own preferences 

纳入标准:

Inclusion criteria

排除标准:

1.婴儿、儿童期肾病综合征发作; 2.继发型膜性肾病(例如系统性自身免疫性疾病、实体或血液恶性肿瘤、慢性乙型病毒感染及其他感染)或长期服用药物(金盐、非甾体类抗炎药[NSAIDs]、青霉胺等); 3.有恶性肿瘤病史; 4.目前正在进行肾脏透析或预期在研究期间需要透析; 5.肝功能异常,谷丙转氨酶或谷草转氨酶超过正常值上限3倍; 6.各种原因(如听力障碍、语言障碍等)造成的无法配合研究人员完成基本的信息采集; 7.其他原因造成的研究人员认为不适合参与研究的情况(如配合意愿低、长期随访困难)。

Exclusion criteria:

1.Onset of nephrotic syndrome in infants and children. 2.Secondary membranous nephropathy (e.g., associated with systemic autoimmune diseases, solid or hematologic malignancies, chronic hepatitis B virus infection, and other infections) or prolonged use of certain medications (e.g., gold salts, nonsteroidal anti-inflammatory drugs [NSAIDs], penicillamine). 3.History of malignant tumors. 4.Currently undergoing kidney dialysis or expected to require dialysis during the study. 5.Abnormal liver function, with alanine aminotransferase (ALT) or aspartate aminotransferase (AST) levels exceeding three times the upper limit of normal. 6.Inability to cooperate with researchers in basic information collection due to various reasons (e.g., hearing impairment, language barriers). 7.Other reasons that the researcher deems unsuitable for participation in the study (e.g., low willingness to cooperate, difficulties in long-term follow-up).
