研究目的：

重要慢性疾病包括肥胖、糖尿病、心血管疾病等。慢性病病因复杂、病程长，显著影响人们身体健康。随着人民生活水平的提高和生活方式的改变，慢性疾病的患病率也逐年上升。本研究总体目标是以肥胖和糖尿病为切入点，建立肥胖、糖尿病等慢性病的风险评估模型，发展有针对性的慢性病行为危险因素干预适宜技术，并在社区慢性病防治中进行应用，为最终建立适合于我国人群的慢性病风险评估体系以及社区慢性病综合防治提供技术支持和服务提供理论依据。

Objectives of Study：

Main chronic diseases include obesity, diabetes, and cardiovascular disease etc. Chronic diseases have complex etiology and a long course, and have great influence on peoples health. With the change of peoples living standard and life style, the number of chronic disease is increasing year by year. The aim of this study is to establish a chronic disease risk assessment model using the obesity and diabetes as the entry point, hoping to develop a suitable intervention technique which targets chronic disease risk factors and could be applied in the community prevention, and provide theory basis of technical support and services for the final chronic disease risk assessment system and the prevention and control of chronic diseases in Chinese population.

Inclusion criteria

排除标准：

(1) 不在1951-1975年出生的
(2) 未住满1年或者已迁出
(3) 有严重的心、肺、肝、肾功能疾病
(4) 活动受限，不能独立参加项目，非自愿参加项目

Exclusion criteria：

(1) participants were not born in 1951-1975;
(2) have been living in the corresponding area less than one year, or move out;
(3) had serious diseases of heart, lung, liver, and kidney;
(4) had obvious activity limitation, can not participated independently and involuntary.
