研究目的:

本研究主要目的是通过双盲随机对照试验,探究靶向初级运动皮层(primary motor cortex, M1)的经颅随机噪声刺激结合直流偏移(tRNS + DC-offset)干预的镇痛效应及镇痛机制,同时探究不同参数水平的tRNS的镇痛效果差异。本研究旨在探索靶向M1的tRNS + DC-offset的治疗潜力及其对人类实验性疼痛模型的疼痛感知(疼痛敏感性、疼痛强度、疼痛不愉悦)、情绪和认知的可能影响。同时通过将心理物理学评估和神经调节技术与实验性疼痛模型相结合,检查tRNS+DC-offset在大脑皮质驱动的镇痛途径中调节大脑可塑性的可行性及其背后神经生理机制。对疼痛敏感性脑机制进行持续的研究将有效促进疼痛心理物理模型的建立,有利于深刻理解疼痛,并以此不断优化疼痛管理方案,给疼痛人群提供了一种多角度综合的康复治疗手段。  

Objectives of Study:

The main purpose of this study is to explore the analgesic effect and analgesic mechanism of tRNS + DC-offset intervention targeting Primary Motor Cortex (M1) through a double-blind randomized controlled trial, and to explore the differences in the analgesic effects of tRNS at different parameter levels. This study aimed to explore the therapeutic potential of tRNS + DC-offset targeting M1 and its possible effects on pain perception (pain sensitivity, pain intensity, pain unpleasantness), emotion, and cognition in human experimental pain models. At the same time, by combining psychophysical assessment and neuromodulation techniques with experimental pain models, we examine the feasibility of tRNS+DC-offset in regulating brain plasticity in the cerebral cortex-driven analgesic pathway and the neurophysiological mechanisms behind it. Continuous research on the brain mechanism of pain sensitivity will effectively promote the establishment of a psychophysical model of pain, which is conducive to a deep understanding of pain, and thereby continuously optimize pain management programs, providing a multi-angle and comprehensive rehabilitation treatment method for people with pain.

Inclusion criteria

排除标准:

(1)女性处于月经期或妊娠期; (2)近3个月内接受过经颅电刺激或经颅磁刺激等神经调控干预; (3)头部有金属植⼊物或体内有心脏支架、起搏器等金属植⼊物; (4)抑郁症、精神分裂症或自闭症等精神疾病,定期使用阿片类药物或抗精神病药物; (5)任何神经系统或肌肉骨骼疾病,尤其是癫痫、中风、脑外伤或偏头痛的病,以及影响感觉处理疾病,如糖尿病等; (6)实验当天存在其他急性症状,如感冒、发烧等。

Exclusion criteria:

(1) Women are in menstruation or pregnancy; (2) Received neuromodulation intervention such as transcranial electrical stimulation or transcranial magnetic stimulation within the past 3 months; (3) There are metal implants on the head or metal implants such as heart stents and pacemakers in the body; (4) Mental illness such as depression, schizophrenia or autism, regular use of opioids or antipsychotics; (5) Any neurological or musculoskeletal disease, especially epilepsy, stroke, brain trauma or migraine, as well as diseases that affect sensory processing, such as diabetes; (6) There are other acute symptoms on the day of the experiment, such as colds, fever, etc.

DC current during sham stimulation was also increased in a ramp-like manner for 30 s until reaching 1 mA. The current intensity then gradually decreases (ramps off) over 30 seconds until it switches off (0 mA). This was followed by 20 minutes of no active stimulation. Therefore, the sham stimulation duration was as long as the real tRNS intervention, facilitating blinding.

受试者随机分组序列有专门的统计人员采用 SPSS 24.0 (IBM, Chicago, IL, USA)统计软件程序产生,并应用不透光、密闭封信法隐藏随机分组序列。将合格对象按照纳入的顺序编号以及 1:1:1:1的比例随机分配进入4种刺激参数分组,采用字母A、B、C、D 代替组别,由课题组专人进行协议和分组匹配,研究人员完全按照分组进行试验协议选择无法得知协议设置内容。

Randomization Procedure (please state who generates the random number sequence and by what method):

The random grouping sequence of subjects was generated by specialized statisticians using the SPSS 24.0 (IBM, Chicago, IL, USA) statistical software program, and the random grouping sequence was hidden using an opaque and sealed letter method. Eligible subjects will be randomly assigned into four stimulus parameter groups according to the order of inclusion and the ratio of 1:1:1:1. The letters A, B, C, and D will be used to replace the groups. The research team will conduct protocol and group matching. The researcher selects the experimental protocol completely according to the grouping and has no way of knowing the content of the protocol settings.

盲法:

本研究采用双盲法,对受试者、研究人员、结局评价人员及统计分析人员设盲。试验结束后,试验分组通过 SPSS 24.0 软件设置盲底,对盲底文件设置密码并由随机序列保管员保管。最终数据统计分析完成后,由随机序列保管员揭底.

Blinding:

This study adopted a double-blind method, blinding the subjects, researchers, outcome evaluators, and statistical analysts. After the experiment, the experimental groups set the blind bottom through SPSS 24.0 software, set a password for the blind bottom file and kept it by the random sequence custodian. After the final statistical analysis of the data is completed, the random sequence keeper will reveal the bottom.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

1、数据采集通过线下的纸质版表格与问卷进行采集。 2、数据将由研究人员上传为电子版本进行储存,纸质版将保存在有锁的档案柜中,仅供研究人员查阅。电子版数据还将单独存储记录于U盘里,由负责人保管,只有研究主要人员和负责人可以接触,储存期限为3年,3年后所有数据将被销毁,永久删除U盘内容。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

1. Data collection is done through offline paper forms and questionnaires. 2. The data will be uploaded by researchers as electronic versions for storage, and the paper versions will be kept in locked filing cabinets for researchers only. The electronic data will also be separately stored and recorded in a USB flash drive, which will be kept by the person in charge. Only the main research personnel and the person in charge can access it. The storage period is 3 years. After 3 years, all data will be destroyed and the contents of the USB flash drive will be permanently deleted.
