主要目的:本项目拟对 iTR-LSVS 手术患者应用三维超声心动图测量缩流颈与三尖瓣环面积比值并进行成比例/不成比例三尖瓣反流分类,评估该分类与患者手术预后的关系。 次要目的:探索基于三维超声心动图测量缩流颈与三尖瓣环面积比值的成比例/不成比例三尖瓣反流患者间右心功能、右心-肺动脉偶联状态、三尖瓣复合体结构的差异;探索该分组与术后右心重构间的联系。  

Objectives of Study:

The primary objective of this project is to apply three-dimensional echocardiography in patients undergoing iTR-LSVS surgery, and measure the ratio of VCA to the tricuspid annulus area, categorize tricuspid regurgitation as either proportional or disproportional, investigate the prognostic value of this categorization. The secondary objective is to explore differences in right heart function, right heart-pulmonary artery coupling, and tricuspid valve complex structure between patients with proportional and disproportional tricuspid regurgitation. Additionally, the project aims to investigate the relationship between this categorization and postoperative right heart remodeling.

Inclusion criteria

排除标准:

1.既往冠心病史,包括心绞痛、心肌梗死、冠脉搭桥术后或冠脉支架术后; 2.肺栓塞或慢性阻塞性肺疾病; 3.三尖瓣或肺动脉瓣狭窄; 4.轻度以上肺动脉瓣反流; 5.中度及以上左心瓣膜病变; 6.左心室射血分数小于40%; 7.心超估测肺动脉收缩压大于55mmHg; 8.预期寿命<1 年,或预计不能完成随访者; 9.超声图像不清晰。

Exclusion criteria:

1. History of coronary artery disease, including angina, myocardial infarction, post-coronary artery bypass graft surgery, or post-coronary stent implantation. 2. Pulmonary embolism or chronic obstructive pulmonary disease. 3. Tricuspid or pulmonary valve stenosis. 4. Mild or greater pulmonary valve regurgitation. 5. Moderate or greater left heart valve disease; 6. Left ventricular ejection fraction less than 40%; 7. Echocardiographic estimation of pulmonary artery systolic pressure greater than 55mmHg; 8. Expected lifespan less than 1 year, or anticipated inability to complete follow-up; 9. Unclear echocardiographic images.

数据采集和管理(说明:数据采集和管理由两部分组成,一为病例记录表(Case Record Form, CRF),二为电子采集和管理系统(Electronic Data Capture, EDC),如ResMan即为一种基于互联网的EDC:

研究者根据入组研究对象的实际情况,按照要求如实填写 CRF,并且通过审 核声明签字后方可生效。已经审核声明签字的 CRF 交数据管理员,CRF 的数据由 两人配合录入至数据库中,一人读取数据,一人电脑录入,双人同时审核的方法 进行。由统计人员随机抽取若干份 CRF 和已录入的数据进行人工比较,以确保已录入的数据与CRF 中的结果一致。研究者必须妥善处理所有临床研究过程中获得的数据,以保证参与临床研究的受试者的权利。在所有的疑问表返回后,将对数据进行最后的修改。对于所有的修改及更新都需要进行记录及归档。在数据完全核实后,召开数据管理会议,对关键数据的修改及更新情况进行总结。由数据管理人员宣布数据的锁定并保存密码。数据锁定后,统计分析计划将不再更改。数据将被转到统计人员供统计分析使用。数据库锁定后的数据可以进行分析和撰写文章。

Data collection and Management (A standard data collection and management system include a CRF and an electronic data capture:

The researchers should fill out the Case Report Form (CRF) truthfully according to the actual situation of the subjects enrolled in the study and it becomes effective only after being reviewed and signed. The CRFs that have been reviewed and signed are handed over to the data administrator. The data in the CRFs are entered into the database by two people working together: one reads the data, and the other enters it into the computer, with both reviewing the data simultaneously. Statisticians randomly select a number of CRFs and the data already entered for manual comparison to ensure the data entered matches the results in the CRFs. Researchers must properly handle all data obtained during the clinical study to protect the rights of the participants. After all query forms are returned, final modifications will be made to the data. All changes and updates need to be recorded and archived.
