探讨多学科共同管理在县域创伤中心老年髋部骨折诊治中的有效性及可行性

Objectives of Study：

To explore the effectiveness and feasibility of multi-disciplinary co-management in the treatment of older hip fractures in county trauma center

药物成份或治疗方案详述：

开通老年髋部骨折绿色通道，患者进入急诊后，干预组即由接诊的急诊医生启动多学科共同管理模式。由急诊科医生护士、骨科医生协同评估患者病情,完善必要检查。入院后由骨科医生、护士及老年病科医生共同管理患者，每日共同查房，并对患者进一步的评估。可根据患者具体病情请其他科室医生一同介入，并及时请麻醉科协助术前准备，尽早完成患者病情评估及处理，尽早手术。术后由骨科医生护士、老年病科医生、麻醉科医生、康复科医生共同管理。对照组患者由门、急诊收治入院，在骨科病房由骨科医生进行全身评估，如有异常，再邀请相关专科医生会诊，处理相关基础疾病。待基础疾病稳定后，再经麻醉科作术前评估后进行手术。术后按以往常规由骨科医生护士进行相关诊治。所有患者出院后主要由骨科医生护士进行共同管理

Description for medicine or protocol of treatment in detail：

Establish a green channel for elderly hip fracture patients. Upon admission to the emergency department, the intervention team, initiated by the attending emergency physician, implements a multi-disciplinary co-management model. Emergency department physicians and nurses, along with orthopedic physicians, collectively assess the patient's condition and conduct necessary examinations. Following hospitalization, orthopedic physicians, nurses, and geriatric physicians jointly manage the patient, conducting daily rounds and further assessments. Other specialists may be consulted based on the patient's specific condition, and prompt involvement of anesthesiologists for preoperative preparation is ensured to expedite the patient's condition evaluation and treatment, facilitating early surgery. Postoperatively, the patient is jointly managed by orthopedic physicians, nurses, geriatric physicians, anesthesiologists, and rehabilitation physicians. In the control group, patients admitted through the emergency department are assessed holistically by orthopedic physicians, and if abnormalities are identified, relevant specialists are consulted to address underlying medical conditions. Once the underlying medical conditions are stabilized, preoperative assessment is conducted by the anesthesiology department prior to the surgery. Postoperatively, the patient receives customary diagnosis and treatment primarily from orthopedic physicians and nurses. Upon discharge, all patients are mainly managed by orthopedic physicians and nurses

纳入标准：

排除标准：

1.骨折部位≥2处； 2.病理性骨折； 3.采用保守治疗; 4.入院前已经有下肢深静脉血栓病史。 5.同侧髋部曾有外伤并行内固定手术

Exclusion criteria：

1. Fractures involving two or more anatomical sites 2. Pathological fractures 3. Conservative treatment adopted 4.History of deep vein thrombosis in the lower limbs before admission 5. Previous trauma and internal fixation surgery on the same side of the hip.
