研究目的：

探索 60 岁及以上糖尿病患者多重心血管危险因素异常管理平台，实施血糖、血脂和血压药物干预和科学评价，为落实糖尿病患者的冠心病三级预防提供质量控制和防控策略建议。  

Objectives of Study：

To explore a target-driven management for multi-cardiovascular risk factors in patients with diabetes mellitus, who aged 60 and over. To apply pharmacologic therapy and evaluate target control of blood glucose, lipids and blood pressure. To provide strategy and recommendation for primary to tertiary prevention of coronary heart diseases in patients with diabetes.

纳入标准：

Inclusion criteria

排除标准：

1. 发生糖尿病酮症酸中毒；2. 有急性心脑血管疾病；3. 患者无法按要求随访。

Exclusion criteria：

1. Diabetic ketoacidosis; 2. Acute cardiovascular and cerebrovascular diseases; 3. The patient can not visit clinic for follow-up as study protocol.

依据本项目血糖、血脂和血压异常达标控制标准要求，严格按照随访计划完成观察指标信息采集、危险因素监测以及危险因素干预达标控制要求内容。药物干预是血糖、血脂和血压达标控制的重要方法。根据国内外临床诊治指南建议，对心血管获益或无害的降糖药，如中国2型糖尿病防治指南（2020年版）建议的双胍类降糖药为2型糖尿病患者血糖控制一线用药和联合药物中的基本用药，可使糖化血红蛋白（HbA1c）下降1.0-1.5%，伴体重下降；降脂药首选他汀类药物，降低低密度脂蛋白胆固醇。如低密度脂蛋白胆固醇（LDL-C）≥3.6mmol/L，选择瑞舒伐他汀或阿托伐他汀，可以降低50%以上。如LDL-C<3.6mmol/L，选择其他他汀药，可以降低30-49%；降压药首选ACEI或ARB类，使糖尿病患者获益更多。

干预措施代码：

Intervention：

A target-driven management for blood glucose, lipids and blood pressure in accordance with study protocol. Patients receive intensive therapy, including tight follow-up, collection of related indicators, monitoring risk factors, and intervene in risk factors to meet the target control requirements. Pharmacologic therapy plays an significant role in achieving the treatment goal of blood glucose, lipids, and blood pressure. According to the recommendations of national and international clinical diagnosis and treatment guidelines, doctors in research team use hypoglycemic drugs that benefit or to be harmless to patients with cardiovascular diseases. Such biguanides as metformin is recommended in guideline for the prevention and treatment of type 2 diabetes mellitus in China (2020 edition) , that are the first-line and basic treatment for glucose control in type 2 diabetes. Biguanides can reduce blood glycosylated hemoglobin (HbA1c) by 1.0-1.5% and result in weight loss. Statins are preferred as lipid-lowering agents to reduce low-density lipoprotein cholesterol (LDL-C). If LDL-C ≥ 3.6mmol/L, rosuvastatin or atorvastatin can be used, and LDL-C decreased by more than 50%. If LDL-C<3.6mmol/L, the other statins can be prescribed for LDL-C reduction by 30-49%. ACEI or ARB is recommended as the first choice of antihypertensive drugs, which benefits patients with diabetes.
